目的 研究氢饱和生理盐水(氢水)对SD大鼠心肌缺血再灌注损伤的保护作用.方法 健康成年雄性SD大鼠36只,按数字表法随机分为假手术组、手术对照组(对照组)、氢水治疗组(氢水组),每组12只.假手术组只开胸,不做缺血再灌注处理;对照组和氢水组在冠状动脉左前降支阻断血流30 min后行再灌注24 h,造成心肌缺血再灌注损伤,氢水组再灌注5 min前以5 ml/kg腹腔注射氢水.对照组和氢水组再灌注24 h后检测左心室收缩压(LVSP)、左心室舒张压(LVDP)、左心室内压最大上升和下降速率[±(dP/dt) max]等心功能指标;用氯化三苯基四氮唑(TTC)染色观察心肌梗死面积,HE染色观察心肌损伤程度,并检测血浆和心肌中丙二醛(MDA)的表达以观察心肌氧化损伤程度,用TUNEL法检测心肌缺血区(area at risk,AAR)心肌细胞凋亡,分光光度法检测心肌凋亡酶caspase-3的表达,免疫组化检测心肌8-羟基鸟嘌呤(8-OHdG)的表达.假手术组在同时间点用相同方法观察或检测上述指标.结果 与对照组相比,氢水组心肌缺血再灌注损伤显著减轻,心肌组织病理性损伤减轻,脂质 和DNA过氧化损伤指标明显减低,心功能显著改善,心肌细胞凋亡减轻,caspase-3表达降低(氢水组0.278±0.092,对照组0.507±0.072),心肌梗死面积显著减少.结论 氢水可以有效减少心肌缺血再灌注损伤.

Objective To investigate the protective effect of hydrogen-saturated saline on the injury induced by reperfusion following cardiac ischemia in Sprague-Dawley rats.Methods Thirty-six male SD rats were randomly divided into the sham surgery group,the control group and hydrogen-saturated saline treatment group,each consisting of 12 animals.The animals in the sham surgery group received thoracic surgery,but did not have reperfusion.Following 30-minute occlusion of blood flow in the left anterior descending ( LAD )coronary artery,24-hour reperfusion was performed in the animals of the control group and hydrogen-saturated saline group,inducing reperfusion injury following cardiac ischemia.The hydrogen group received abdominal injection of hydrogen-saturated saline at a dosage of 5 ml/kg,5 minutes before reperfusion.Cardiac functions,such as left ventricular systolic pressure (LVSP),left ventricular diastolic pressure (LVDP),maximal increasing and decreasing rates of left intraventricular pressures [ ±(dP/dt) max ] were monitored in the animals of the control group and the hydrogen saline group,24 h after reperfusion.The size of cardiac infarct was assessed with triphenyltetrazolium chloride (TTC) staining.Morphological injury was measured with Hematoxylin and Eosin (HE) staining.The expressions of malondialdehyde (MDA) in the plasma and myocardium were measured to see the oxidative stress of myocardium.Apoptosis of cardiac cells in the affected area was measured with TUNEL.The expression of caspase-3 was monitored with the spectrophotometric method,and the expression of 8-hydroxy-desoxyguanosine (8-OHdG) in the affected area was measured with immunohistology.For the animals in the sham surgery group,above indices were assessed or monitored at the same time points and with the same methods.Results When compared with the control group,following changes could be noticed:reperfusion injury for the animals of the hydrogen saline group improved considerably,pathological lesion in the affected cardiac tissue lessened,indices of lipid and DNA oxidation decreased significantly,cardiac function improved considerably,apoptosis of cardiac muscular cells decreased,the expression of caspase-3 was reduced and the size of cardiac infarction decreased significantly.Conclusions Hydrogen - saturated saline could effectively alleviate reperfusion injury following cardiac ischemia.
