目的 探讨低氧诱导因子-1α(HIF-1α)、血管内皮生长因子-C(VEGF-C)在非小细胞肺癌(NSCLC)中的表达及其与淋巴转移的关系.方法 采用PV-9000免疫组织化学法检测58例NSCLC和20例癌旁对照肺组织中HIF-1α、VEGF-C的表达情况,并记数VEGFR-3标记淋巴管内皮细胞的微淋巴管密度(LMVD).结果 HIF-1α和VEGF-C在NSCLC组的阳性表达率分别为65.5%和60.3%,明显高于癌旁对照肺组织 (P <0.05);NSCLC组织中HIF-1α和VEGF-C的表达的高低与有无淋巴结转移、TNM临床分期密切相关(P<0.05),而与性别、年龄、病理组织类型以及肿瘤的分化程度无显著相关(P >0.05);LMVD在NSCLC组和癌旁对照组计数分别为6.6±2.4和2.9±1.7,差异有统计学意义(P<0.05);HIF-1α与VEGF-C的表达呈明显相关(rpearson=0.464,P <0.05),HIF-1α与LMVD呈正相关(rs = 0.506,P <0.05),VEGF-C与LMVD也呈正相关(rs = 0.608,P <0.05).结论 HIF-1α及其靶基因VEGF-C在NSCLC中的表达明显上调,HIF-1α可能通过诱导VEGF-C的过表达参与NSCLC的淋巴管生成,在NSCLC的发生、发展以及转移过程中发挥着重要作用.

목적 탐토저양유도인자-1α(HIF-1α)、혈관내피생장인자-C(VEGF-C)재비소세포폐암(NSCLC)중적표체급기여림파전이적관계.방법 채용PV-9000면역조직화학법검측58례NSCLC화20례암방대조폐조직중HIF-1α、VEGF-C적표체정황,병기수VEGFR-3표기림파관내피세포적미림파관밀도(LMVD).결과 HIF-1α화VEGF-C재NSCLC조적양성표체솔분별위65.5%화60.3%,명현고우암방대조폐조직 (P <0.05);NSCLC조직중HIF-1α화VEGF-C적표체적고저여유무림파결전이、TNM림상분기밀절상관(P<0.05),이여성별、년령、병리조직류형이급종류적분화정도무현저상관(P >0.05);LMVD재NSCLC조화암방대조조계수분별위6.6±2.4화2.9±1.7,차이유통계학의의(P<0.05);HIF-1α여VEGF-C적표체정명현상관(rpearson=0.464,P <0.05),HIF-1α여LMVD정정상관(rs = 0.506,P <0.05),VEGF-C여LMVD야정정상관(rs = 0.608,P <0.05).결론 HIF-1α급기파기인VEGF-C재NSCLC중적표체명현상조,HIF-1α가능통과유도VEGF-C적과표체삼여NSCLC적림파관생성,재NSCLC적발생、발전이급전이과정중발휘착중요작용.
Objective To investigate the expression of HIF-1α、VEGF-C and their relation to lymphangiogenesis and lymph node metastasis in non small cell lung cancer(NSCLC).Methods The expressions of HIF-1α and VEGF-C in 58 cases of NSCLC tissues and 20 cases of the adjacent tissues were detected by PV-9000 immunohistochemical technique.LMVD was determined by VEGFR-3 immunostaining.Results The positive rate of HIF-1α and VEGF-C expression in NSCLC was 65.5% and 60.3%,which is significantly higher than that in the adjacent tissues (P<0.05).There was a significant difference of HIF-lα and VEGF-C protein expression between NSCLC tissues and the adjacent tissues (P<0.01).The expression of HIF-1α and VEGF-C were both correlated with lymph node involvement (P<0.05) and TNM clinical stage (P<0.05),but no significant correlation between sex,age,pathological type and differentiation(P >0.05).The expression of HIF-1α was correlated with VEGF-C expression and LMVD(P<0.05).Conclusion Over expression of HIF-1α induces the tumor lymph angiogenesis in NSCLC by activating the transcription of VEGF-C gene.It may play an important role in the generation,development and lymphatic metastasis in NSCLC.
