目的 探讨直肠间叶性梭形细胞肿瘤的诊断与鉴别诊断和预后指标。方法 在光镜观察的基础上,运用免疫组化方法检测15例直肠间叶性梭形细胞肿瘤的Vimentin、Actin、Desmin、S-100蛋白和CD34的表达。结果 病理学确诊良性2例,恶性13例,肿瘤平均直径6.8cm,平均核分裂数8个/10HPF,并随Ⅰ、Ⅱ、Ⅲ分级增高而增多;免疫组化染色证实为平滑肌肿瘤11例(良性2例,恶性9例),GIST4例;随访13例恶性肿瘤中有3例复发,6例死亡,均为Ⅱ、Ⅲ级,其平均核分裂数12个/10HPF。结论 肿瘤大小、核分裂数为直肠平滑肌肿瘤和GIST良恶性诊断的重要指标;免疫组化染色有助于两者的鉴别诊断;瘤细胞异型性、核分裂多少与其预后有关。

목적 탐토직장간협성사형세포종류적진단여감별진단화예후지표。방법 재광경관찰적기출상,운용면역조화방법검측15례직장간협성사형세포종류적Vimentin、Actin、Desmin、S-100단백화CD34적표체。결과 병이학학진량성2례,악성13례,종류평균직경6.8cm,평균핵분렬수8개/10HPF,병수Ⅰ、Ⅱ、Ⅲ분급증고이증다;면역조화염색증실위평활기종류11례(량성2례,악성9례),GIST4례;수방13례악성종류중유3례복발,6례사망,균위Ⅱ、Ⅲ급,기평균핵분렬수12개/10HPF。결론 종류대소、핵분렬수위직장평활기종류화GIST량악성진단적중요지표;면역조화염색유조우량자적감별진단;류세포이형성、핵분렬다소여기예후유관。
Objective To explore the diagnostic and prognostic criteria ofmesenchymal spindle cell tumor of rectum.Methods In addition to light microscopic observations,the expression of Vimentin,Actin,Desmin,S-100 and CD34 were detected by immunohisto-chemical stain in 15 cases of mesenchymal spindle cell tumor of rectum.Results Out of 15 cases tumors,2 cases were benign tumors and 13 cases were malignant tumors.The average diameter of tumors was 6.8cm.The average mitotic count was 8/10HPF,and it increased with grade Ⅰ,Ⅱand Ⅲ.11 cases were smooth muscle tumors and 4 cases were GISTs by immunohistochemical detection.Follow-up date revealed that there are 3 cases of recurrence and 6 cases of death.These tumors were grade Ⅱ and Ⅲ,and their average mitotic count was 12/10HPF.Conclusion The size of tumor and mitotic count play the important role in diagnosis of benign and malignant tumors.Immunohistochemical stains are useful for differentiated diagnosis of smooth muscle tumor and GIST.The cellular pleomorphism and mitotic count are relation to their prognosis.
