目的 了解青少年近视患者高阶像差的分布规律,探讨近视程度对高阶像差的影响.方法 前瞻性系列研究.对2007年8月至2007年9月在山东大学附属省立医院眼科中心应用iTrace视功能分析仪进行检测的青少年近视104例104只眼[年龄(19.9±1.34)岁,等效球镜(-4.10±2.44)D]的高阶像差进行分析.在6mm瞳孔直径下测量3-6阶Zernike系数,计算各阶均方根(RMS)值、3~6阶总RMS值、球差(Z40,Z60)及彗差(Z3-1,Z31,Z5-1,Z51)RMS值,并分别与屈光度进行相关分析.结果 受试者球镜平均为(-3.72±2.36)D,柱镜平均为(-0.77±0.62)D,总高阶像差(3~6阶)的RMS值平均为(0.433±0.176)μm,3阶至6阶的各项Zernike系数大致呈递减趋势(4阶球差Z40除外).全眼总高阶像差RMS值、3~6阶各阶RMS值和彗差RMS值均与近视程度显著相关(P<0.01),而球差和Z60值不随屈光度变化.结论 在青少年近视中,高阶像差的个体差异较大.随着近视屈光度的增加,总高阶像差和彗差呈增加趋势,使视觉成像质量下降.

Objective To investigate the distribution of high-order aberrations (HOAs, 3rd to 6th orders) in myopic eyes of young adults, and study the changes as a function of the degree of myopia. Methods Prospective studies. Using the iTrace Visual Function Analyzer, root mean square (RMS) of total HOAs, 3rd, 4th, 5th-, 6th- order aberrations, coma, and SA were computed from the central 6.0 mm zone of 104 eyes of 104 subjects (mean age 19.9±1.34years; mean manifest spherical equivalent -4.10±2.44D) in the Eye Center of Shandong Provincial Hospital Affiliated to Shandong University from August 2007 to September 2007. Correlation analysis was performed to assess the association between HOAs and refractive power. Results The mean refractive error was sphere -3.72±2.36 D and cylinder -0.77±0.62 D. The mean of the total high order RMS (3rd to 6th) was (0.433±0.176)μm.The values of Zemike coefficients progressively decreased from the 3rd-order to the 6th-order(ex-cept Z40). Significant relationships were found between myopia and total ocular high order RMS, as well as coma (P<0.01), while the SAs and (Z40) were stable. Conclusions The ocular high order aberrations vary greatly among young adults. Total ocular high order aberration and coma increase with the degree of myopia, which decline the quality of retinal image.
