目的 分析甲状腺功能亢进症(甲亢)患者外周血白细胞数量及其VCS变化.方法 以LH750全自动血细胞分析仪对60例甲亢患者外周血白细胞检测,比较白细胞及其分类的数量变化;同时进行VCS参数的数据分析.结果 60例甲亢患者单核细胞相对计数和单核细胞绝对计数分别为(9.0±3.1)%、(4.8±1.9)×108/L,均高于对照组的(6.5±2.3)%、(3.6±1.2)×108/L,两组之间差别显著(P<0.01).甲亢患者具有LMR、中性粒细胞V-均值、淋巴细胞C-均值均降低的特征.LMR的敏感性和特异性分别为72.7%、50.8%(cutoff值=4.0).中性粒细胞V-均值、淋巴细胞C-均值对甲亢的诊断具有较高的敏感度(ROC下面积分别为0.629、0.706).结论 甲亢患者外周血单核细胞数量上升,LMR比值降低,白细胞VCS参数有助于临床上甲亢的诊断.

목적 분석갑상선공능항진증(갑항)환자외주혈백세포수량급기VCS변화.방법 이LH750전자동혈세포분석의대60례갑항환자외주혈백세포검측,비교백세포급기분류적수량변화;동시진행VCS삼수적수거분석.결과 60례갑항환자단핵세포상대계수화단핵세포절대계수분별위(9.0±3.1)%、(4.8±1.9)×108/L,균고우대조조적(6.5±2.3)%、(3.6±1.2)×108/L,량조지간차별현저(P<0.01).갑항환자구유LMR、중성립세포V-균치、림파세포C-균치균강저적특정.LMR적민감성화특이성분별위72.7%、50.8%(cutoff치=4.0).중성립세포V-균치、림파세포C-균치대갑항적진단구유교고적민감도(ROC하면적분별위0.629、0.706).결론 갑항환자외주혈단핵세포수량상승,LMR비치강저,백세포VCS삼수유조우림상상갑항적진단.
Objective To analyze the changes of leukocyte and its VCS parameters in peripheral blood of hyperthyroidism patients. Methods Leukocyte in peripheral blood of 60 hyperthyroidism patients were detected by using Beckman Coulter LH750 hematology analyzer, meanwhile classification and VCS parameters of leukocyte were analyzed. Results The relative and absolute counting of monocytes in 60 hyperthyroidism patients were(9.0 ±3.1)% 、(4.8 ± 1.9) × 108/L respectively,which were significantly higher than those of healthy eontrols(P <0.01 ). LMR,neutrophil V means and lymphocyte C means were decreased in hyperthyroidism patients. The sensitivity and specificity of LMR were 72.7% ,50.8% ( cutoff = 4.0) respectively. Neutrophil V means and lymphocyte C means showed a high sensitivity for the diagnosis of hyperthyroidism ( areas under the ROC curves were 0.629、0.706 respectively). Conclusion An increased monocytes and a decreased LMR ration were found in hyperthyroidism patients, which made leukocyte and its VCS parameters contribute to diagnose of hyperthyroidism.
