目的 探讨利用CRRT及CRRT联合血液吸附方法对重症MODS患者血清血管内皮细胞生长因子-1(VCAM-1)的影响.方法 收集重症MODS患者43例,CRRT治疗组28例,联合治疗组15例,分别给予CRRT及CRRT联合血液吸附治疗.收集治疗前后血清,用ELISA的方法检测患者血清VCAM-1水平,并对患者行APACHEⅡ评分.结果 CRRT组治疗前sVCAM-1(1865.96±726.77)ng/ml,治疗后sVCAM-1(1783.55±868.92)ng/ml;联合组治疗前sVCAM-1(1958.27±932.14)ng/ml,治疗后sVCAM-1(1259.67±687.35)ng/ml,与治疗前比较差异有极显著性(P<0.01).结论 sVCAM-1可作为评价重症MODS患者严重程度的指标且CRRT联合血液吸附明显改善MODS患者内皮细胞功能.

Objectives To study the effect of continous veno-venous hemofiltration with blood absorbtion on sVCAM - 1. Methods 43 Patients with multiple organ dysfunction syndrome (MODS) were divided into two groups:group A(n = 28) received continous veno-venous hemofiltration,group B(n= 15) received continous veno-venous hemofiltration with hemo-absorbtion. Before and three times after therapy, detect serum VCAM-1 levels with ELISA method, and to count up APACHE Ⅱ score for patients. Results Serum VCAM- 1 of severe MODS patients were significantly elevated. sVCAM-1 correlated with APACHE Ⅱ score significantly, (r = 0.548, P<0.05). 3 After CRRT therapy, sVCAM-1 was not changed compared with before therapy, while after CRRT with blood absorbtion therapy, the level of sVCAM-1 decreased significantly. Conclusion sVCAM-1 can be used as evaluation of the severity of patients with severe MODS. CRRT with blood absorbtion is better in endothelial function of MODS than CRRT.
