目的 对原发性高血压不同血压水平患者进行血清抵抗素(RES)检测,探讨RES水平与高血压的发生、发展之间的关系.方法 选择研究对象共154例,其中原发性高血压患者114例,根据血压水平的高低分为高血压1级组(H1)、高血压2级组(H2)和高血压3级组(H3),正常血压组(H4,对照组)40例,检测各组受检者的RES、血总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)和低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FBS);并计算体质指数(BMI).分析影响RES的因素.结果 (1)4组受检者RES水平间差异有统计学意义(F=8.96,P=0.000),其中H4组较H3组水平低,H1、H2组也较H3水平低,差异有统计学意义 (P=0.000);但H4、H1、H2组两两之间差异无统计学意义(P均>0.05);(2)RES与收缩压(SBP) (r=0.201,P=0.012)、舒张压(DBP)(r=0.290,P=0.000)均呈正相关,与高密度脂蛋白胆固醇(r=-0.172,P=0.033)呈负相关;(3)按逐步引入-剔除法以RES为应变量,以年龄(AGE)、BMI、SBP、DBP、TC、TG、HDL-C、LDL-C、FBS为自变量进行多元线性回归分析,RES与DBP、HDL-C之间呈直线关系,DBP、HDL-C是影响RES水平的独立危险因素;以 SBP、DBP分别为应变量,RES、TC、FBS及AGE是影响 SBP、DBP的独立危险因素.结论 (1)血清RES水平在正常血压人群要明显比原发性高血压人群低;高血压1级人群水平要明显比高血压3级人群低.(2)血清RES水平与收缩压、舒张压呈正相关,与HDL-C呈负相关;(3)血清RES的升高是原发性高血压发生和发展进程中的重要影响因素.

목적 대원발성고혈압불동혈압수평환자진행혈청저항소(RES)검측,탐토RES수평여고혈압적발생、발전지간적관계.방법 선택연구대상공154례,기중원발성고혈압환자114례,근거혈압수평적고저분위고혈압1급조(H1)、고혈압2급조(H2)화고혈압3급조(H3),정상혈압조(H4,대조조)40례,검측각조수검자적RES、혈총담고순(TC)、삼선감유(TG)、고밀도지단백담고순(HDL-C)화저밀도지단백담고순(LDL-C)、공복혈당(FBS);병계산체질지수(BMI).분석영향RES적인소.결과 (1)4조수검자RES수평간차이유통계학의의(F=8.96,P=0.000),기중H4조교H3조수평저,H1、H2조야교H3수평저,차이유통계학의의 (P=0.000);단H4、H1、H2조량량지간차이무통계학의의(P균>0.05);(2)RES여수축압(SBP) (r=0.201,P=0.012)、서장압(DBP)(r=0.290,P=0.000)균정정상관,여고밀도지단백담고순(r=-0.172,P=0.033)정부상관;(3)안축보인입-척제법이RES위응변량,이년령(AGE)、BMI、SBP、DBP、TC、TG、HDL-C、LDL-C、FBS위자변량진행다원선성회귀분석,RES여DBP、HDL-C지간정직선관계,DBP、HDL-C시영향RES수평적독립위험인소;이 SBP、DBP분별위응변량,RES、TC、FBS급AGE시영향 SBP、DBP적독립위험인소.결론 (1)혈청RES수평재정상혈압인군요명현비원발성고혈압인군저;고혈압1급인군수평요명현비고혈압3급인군저.(2)혈청RES수평여수축압、서장압정정상관,여HDL-C정부상관;(3)혈청RES적승고시원발성고혈압발생화발전진정중적중요영향인소.
Objective To explore the relationship between essential hypertension and resistin levels.Methods Totally 154 outpatients including 40 patients with normal blood pressure (H4 group) and 114 patients with essential hypertension (EH group) were enrolled in this study.The EH group were chopped into 3 groups according to the blood pressure levels: H1 group (Grade 1 hypertension),H2 group (Grade 2 hypertension),and H3 group (Grade 3 hypertension.Serum resistin,blood cholesterol (TC),trigcylglycerol (TG),high-density lipoprotein (HDL),low-density lipoprotein (LDL),and fasting plasma glucose were examined in each group.Body mass index (BMI) was.calculated.Factors that may influence the resistin levels were analyzed.Results The resistin level of H3 group was significantly higher than that in H4 group (F= 8.959,P=0.000),and also significantly higher than those in H1 and H2 groups (P=0.000).However,such difference was not found between H4,H1,and H2 groups (all P>0.05).(2) Resistin levels were positively correlated with systolic blood pressure (SBP) (r= 0.201,P=0.012) and diastolic blood pressure (DBP) (r= 0.290,P=0.000),and were negatively correlated with high-density lipoprotein cholesterol (r=- 0.172,P=0.033).By using the gradual introduction - excluding method with resistin as the dependent variable and age,BMI,SBP,DBP,TC,TG,HDL,LDL,and FBS as the independent variables,multiple linear regression analysis showed that RES had a linear relation with DBP and HDL,and DBP and HDL are independent factors that affected resistin levels.When using SBP and DBP as the dependent variable seperately,resistin,TC,FBS,and age are independent factors that affected the levels of SBP and DBP.Conclusion Serum resistin levels is lower in normal blood pressure population than in patients with essential hypertension.Serum resistin levels is positively correlated with SBP and DBP and negatively correlated with HDL.The increase of serum resistin play a key role in the occurrence and development of essential hypertension
