目的研究同型半胱氨酸(HCY)与脑梗塞的关系及叶酸、维生素B12(B12)对HCY水平的影响。方法采用高效液相色谱荧光检测法测定40例脑梗塞患者和20例同龄健康对照组血浆HCY浓度,用SimulTRAC-SNB放射法测定两组血清叶酸、B12的含量。结果①脑梗塞患者血浆HCY浓度(24.02*!μmol/L±11.24*!μmol/L)高于对照组(14.0*!μmol/L±4.48*!μmol/L)(P<0.01);②脑梗塞患者叶酸水平(4.82*!μg/L±1.97*!μg/L)显著低于对照组(6.88*!μg/L±2.85*!μg/L)(P<0.001);脑梗塞患者B12含量(247.56*!ng/L±158.72*!ng/L)略低于对照组(270.31*!ng/L±198.39*!ng/L),但无显著差异(P>0.05),Pearson相关分析显示两组HCY浓度与叶酸水平均呈负相关性,与B12水平无显著相关。结论脑梗塞患者血浆HCY浓度高于同龄健康人;HCY浓度与叶酸水平呈负相关性,与B12水平相关性不显著。

목적연구동형반광안산(HCY)여뇌경새적관계급협산、유생소B12(B12)대HCY수평적영향。방법채용고효액상색보형광검측법측정40례뇌경새환자화20례동령건강대조조혈장HCY농도,용SimulTRAC-SNB방사법측정량조혈청협산、B12적함량。결과①뇌경새환자혈장HCY농도(24.02*!μmol/L±11.24*!μmol/L)고우대조조(14.0*!μmol/L±4.48*!μmol/L)(P<0.01);②뇌경새환자협산수평(4.82*!μg/L±1.97*!μg/L)현저저우대조조(6.88*!μg/L±2.85*!μg/L)(P<0.001);뇌경새환자B12함량(247.56*!ng/L±158.72*!ng/L)략저우대조조(270.31*!ng/L±198.39*!ng/L),단무현저차이(P>0.05),Pearson상관분석현시량조HCY농도여협산수평균정부상관성,여B12수평무현저상관。결론뇌경새환자혈장HCY농도고우동령건강인;HCY농도여협산수평정부상관성,여B12수평상관성불현저。
To investigate the plasma homocysteine level and cerebral infarction and the association between plasma homocysteine level and levels of serum folate and vitamin B12.Methods Levels of plasma homocysteine of 40 cerebral infarction patients and 20 healthy subjects were determined using high-performance liquid chromatography with fluorescence detection. Serum folate and vitamin B12concentrations were measured by SimulTRAC-SNB radioassay.Results ①The mean plasma level of homocystine was higher in patients than that in control[(24.02±11.24)*!μmol/L versus(14.10±4.48)*!μmol/L,P<0.01];②The level of serum folate was significantly lower in patients than that in control[(4.82±1.97)*!μg/L versus(6.88±2.85)*!μg/L,P<0.001].The level of serum B12was slightly lower in patients[(247.56±158.72)*!ng/L] than that in control[(270.31±198.39)*!ng/L],but the difference was not statistically significant (P>0.05).Person correlation anslysis revealed that plasma homocysteine level showed a strong inverse association with folate level in patients(r=-0.39,P=0.01)and in controls(r=-0.46,P=0.03).Vitamin B12 did not show correlation to plasma homocysteine in cases or controls(P>0.05).Conclusion ①Plasma homocysteine level in cerebral infarction patients is higher than that in the same age health.②There is a strong inverse association between plasma homocysteine and folate.The association between plasma homocysteine and vitamin B12 is not statistically significant.
