目的 研究严重感染和感染性休克患者急性期动静脉血气分析指标的变化规律及与疾病预后的关系.方法 对我科四年来收治的47例严重感染和感染性休克患者的动脉及中心静脉血气分析进行研究.结果 肺泡气-动脉血氧分压差[P(A-a)O_2]和中心静脉血氧饱和度(ScVO_2)在疾病急性期与缓解期比较差异有统计学意义(P<0.01),疾病缓解后P(A-a)O_2 值下降,ScVO_2值有所升高;而在疾病恶化时两者与急性期比较差异均无统计学意义(P>0.05).结论 P(A-a)O_2及ScVO_2能敏感反映患者病情变化,动静脉血气分析联合监测可以有效判断疾病的预后.

목적 연구엄중감염화감염성휴극환자급성기동정맥혈기분석지표적변화규률급여질병예후적관계.방법 대아과사년래수치적47례엄중감염화감염성휴극환자적동맥급중심정맥혈기분석진행연구.결과 폐포기-동맥혈양분압차[P(A-a)O_2]화중심정맥혈양포화도(ScVO_2)재질병급성기여완해기비교차이유통계학의의(P<0.01),질병완해후P(A-a)O_2 치하강,ScVO_2치유소승고;이재질병악화시량자여급성기비교차이균무통계학의의(P>0.05).결론 P(A-a)O_2급ScVO_2능민감반영환자병정변화,동정맥혈기분석연합감측가이유효판단질병적예후.
Objective To study the regularity and the relationship of blood gas analysis with the prognosis in the patients who have severe sepsis and septic shock at acute exacerbation stage. Methods We studied arterial and central venous blood gas analysis of 47 patients with severe sepsis and septic shock from our ICU in the past two years. Results There were significant differences in alveolar-arterial(A-a) oxygen tension difference [P(A-a)O_2] and central venous oxygen saturation(ScVO_2) at acute exacerbation stage and recovery stage(P<0.01),the P(A-a)O_2 decreased and the ScVO_2 increased during recovery. There were no significant differences in two parameters between acute exacerbation stage and deterioration stage(P>0.05). Conclusion P(A-a)O_2 and ScVO_2 can sensitively reflect the variation of disease, and combined monitoring of arterial and central venous blood gas analysis can effectively predict the prognosis.
