目的 检测不同飞行条件下飞行员飞行训练前后凝血及纤溶指标的变化,以了解飞行训练对其凝血和纤溶平衡的影响. 方法 应用生物活性法和免疫比浊法,检测40名飞行员飞行训练前后的静脉血凝血酶原时间(prothrombin time,PT)、凝血酶时间(thrombin time,TT)、活化部分凝血活酶时间(activated partial thromboplastin time,APTT)、纤维蛋白原(fibrinogen,Fg)和D-二聚体(D-dimer,D-D).依不同飞行强度(1次飞行和两次飞行)、不同飞行场次(昼航和跨昼夜飞行)、不同飞行载荷(+3Gz和+5Gz)、不同飞行高度(3 km和5 km)分别各分为两组.另设30名场站地勤人员为对照组.比较自身飞行训练前后,及不同组间凝血-纤溶指标的差异. 结果 飞行前飞行员组与对照组凝血-纤溶指标的差异无统计学意义.飞行训练后PT、TT、APTT缩短、Fg、D-D升高,分别与飞行前自身及对照组比较,差异有统计学意义(t=2.925~6.908,P<0.01).完成各飞行课目后分别与飞行前比较,PT、TT、APTT缩短,Fg、D-D升高,差异有统计学意义(t=2.348~8.717,P<0.01或P<0.05).第1次飞行后与第2次飞行后、昼航与跨昼夜飞行后、3 km与5 km飞行后比较,凝血-纤溶指标的差异无统计学意义.+5Gz飞行后与+3Gz飞行后比较,TT缩短、Fg升高,差异有统计学意义(t=2.605、5.772,P=0.013、0.000);其他指标差异无统计学意义. 结论 飞行员飞行训练后凝血与纤溶指标可发生改变,属于机体应激状态下的适应性反应.

Objective To measure the changes of coagulation and fibrinolysis indexes of the pilots caused by different flight conditions,and to find out the effects of flight training on the balance between coagulation and fibrinolysis. Methods By using biological activity method and immunoturbidimetry blood prothrombin time (PT), thrombin time (TT), activated partial thromboplastin time (APTT),fibrinogen (Fg) and D-dimer (D-D) were tested on 40 pilots who were respectively divided into 2 pilot groups by such conditions as flying strength (1 or 2 flights),flying time (day or over night flight),acceleration load (+3 Gz or +5 Gz) and flying altitude (3 km or 5 km).Thirty ground crews were chosen as control group.The coagulation and fibrinolysis indexes were self-compared for getting training effects and were also compared between groups. Results Before training pilot groups showed no significant difference with control group on coagulation and fibrinolysis indexes.But training caused significantly shorter PT,TT,APTT and significantly higher Fg and D-D level (t=3.287-6.908,P<0.01),and significantly different with those of control group (t=2.925-5.059,P<0.01). Compared to preflight state,PT,TT and APTT levels were significantly shorter and Fg and D-D levels were higher after flight in each group (t=2.348-8.717,P<0.01 or P<0.05).There was no significant difference in the pilot groups between the conditions of flying strength,flying time and flying altitude.+ 5 Gz flight caused shorter TT and higher Fg level than that of +3 Gz flight (t=2.605,5.772,P=0.013,0.000).Other indexes had no significant difference. Conclusions Flight training may cause the changes on pilot's coagulation and fibrinolysis indexes.It reflects an adaptive response to the body under stress.
