目的 探讨汉族人群中外周血儿茶酚氧位甲基转移酶(COMT)基因表达与强迫症的关系.方法 采用实时定量逆转录-聚合酶链反应技术检测35例强迫症首次发病患者(强迫症组)与31名健康对照(对照组)外周血COMT基因表达水平;使用美国精神障碍诊断与统计手册第4版轴Ⅱ诊断结构式临床访谈问卷(SCID-Ⅱ)评估强迫症与强迫性人格障碍的共病情况.结果 强迫症组COMT基因表达水平明显低于对照组(t=2.56,P<0.05),下调约32%;无强迫人格患者(15例)与对照组间COMT基因表达水平的差异有统计学意义(t=2.41,P<0.05),而伴强迫人格患者(20例)与对照组的差异无统计学意义(t=1.50,P>0.05);伴强迫人格与无强迫人格的强迫症患者COMT基因表达水平分别下调14%和56%;COMT表达水平与症状严重度无显著相关(r=0.09,P<0.05).结论 COMT基因表达下调可能与强迫症发病有关;伴或无强迫人格患者的遗传机制可能存在差异.
目的 探討漢族人群中外週血兒茶酚氧位甲基轉移酶(COMT)基因錶達與彊迫癥的關繫.方法 採用實時定量逆轉錄-聚閤酶鏈反應技術檢測35例彊迫癥首次髮病患者(彊迫癥組)與31名健康對照(對照組)外週血COMT基因錶達水平;使用美國精神障礙診斷與統計手冊第4版軸Ⅱ診斷結構式臨床訪談問捲(SCID-Ⅱ)評估彊迫癥與彊迫性人格障礙的共病情況.結果 彊迫癥組COMT基因錶達水平明顯低于對照組(t=2.56,P<0.05),下調約32%;無彊迫人格患者(15例)與對照組間COMT基因錶達水平的差異有統計學意義(t=2.41,P<0.05),而伴彊迫人格患者(20例)與對照組的差異無統計學意義(t=1.50,P>0.05);伴彊迫人格與無彊迫人格的彊迫癥患者COMT基因錶達水平分彆下調14%和56%;COMT錶達水平與癥狀嚴重度無顯著相關(r=0.09,P<0.05).結論 COMT基因錶達下調可能與彊迫癥髮病有關;伴或無彊迫人格患者的遺傳機製可能存在差異.

Objective To explore the association of peripheral catechol-O-methyl-transferase (COMT)gene expression and obsessive-compulsive disorder(OCD)in Han Chinese.Methods Peripheral blood samples were collected from individuals with OCD(n=35)and age-and sex-matched healthy subjects (n=31).Relative COMT gene expression levels were examined with real-time quantitative reverse transcription PCR in peripheral blood of all the subjects.Structured clinical interview for the 4th Edition of Diagnostic and Statistical Manual of Mental Disorder axisⅡ(SCID-Ⅱ)was used to assess the co-morbidity of OCD and obsessive-compulsive personality disorders(OCPD).The Yale-Brown Obsessive-Compulsive Scale(Y-BOCS)was used to evaluate the severity of obsessive compulsive symptoms.Results The COMT gene expression levels were significantly decreased in the OCD group compared with healthy control group (t=2.56,P<0.05).The OCD patients showed a 32%down-regulation of COMT gene expression.A significant lower COMT gene expression was observed in non-OCPD patients(n=15)in comparison with healthy controls(t=2.41.P<0.05),while there was,no significant difference between OCPD patients (n=20)and healthy controls(t=1.50,P>0.05).The rates of COMT gene expression down-regulation in OCD patients with OCPD and ones without OCPD were 14%and 56%respectively.There was no significant correlation between COMT expression level and the Y-BOCS score(r=0.09,P=0.62).Conclusions These results suggest that COMT gene expression down-regulation is associated with OCD,and the OCD patients with and without OCPD may have different genetic background.
