目的提高肾结核的术前诊断率。方法回顾性分析180例手术治疗的肾结核患者的临床资料。结果膀胱刺激征、血尿和腰痛是最常见的临床症状，无症状的肾结核所占比例为6.1％。尿查抗酸杆菌阳性率为12.4％。IVP、B超、CT、CTU的诊断阳性率分别为23.5％、75％、85.6％、95.3％。误诊肾结石6例，误诊肾积水4例。该组病人均行患侧肾及大部分输尿管切除。结论注意肾结核和肾积水及肾结石的鉴别，重视B超在肾结核诊断中的作用，CTU在肾结核的诊断中价值最大。终合分析临床资料是提高肾结核术前诊断的关键。
목적 제고신결핵적술전진단솔。방법 회고성분석180례수술치료적신결핵환자적림상자료。결과 방광자격정、혈뇨화요통시최상견적림상증상，무증상적신결핵소점비례위6.1％。뇨사항산간균양성솔위12.4％。IVP、B초、CT、CTU적진단양성솔분별위23.5％、75％、85.6％、95.3％。오진신결석6례，오진신적수4례。해조병인균행환측신급대부분수뇨관절제。결론주의신결핵화신적수급신결석적감별，중시B초재신결핵진단중적작용，CTU재신결핵적진단중개치최대。종합분석림상자료시제고신결핵술전진단적관건。
Objectives To improve the diagnosis accuracy for renal tuberculosis before operation. Methods A retrospective study was made on 180 cases after operation. Results Irritation symptoms, hematuria and lumbodynia were the moot common symptoms. The positive result of acid - fast stains, urinary was 12.4％ ;The diagnostic accuracy of intravenous urography( IVP), B - type ultrasonography ,CT and CTU were 23.5％ ,75％ ,85.6％and 95.3％ ,6 cases were misdiagnosed to have calculi ,4 cases were misdiagnosed as hydronephrosis. all cases underwent nephrectomize,the involved ureter was resscted as much as possible. Conclusions Pay attention to the identification with hydronephrosis and calculi. B - type ultrasonography play more role in diagnosis of renal tuberculosis, CTU are the most valuable diagnostic methods. It is the key of the comprehensive analysis of the clinic feature, laboratory and imaging finding.
