目的:探索良好的肠吻合术式,以防止肠闭锁之小口径肠管吻合术后吻合口梗阻的发生.方法:改良肠单层吻合术与全层内翻吻合术进行动物实验比较,并临床应用.结果:切片观察示吻合口粘膜连续性恢复好,粘膜缺损少,粘膜下移少而轻,粘膜下层对合满意.与全层内翻吻合比较有统计学差异.术后2周吻合口标本羟脯氨酸含量(代表胶原含量)实验组与对照组分别为43.527 μg/mg干重和41.423μg/mg干重(P>0.05).7例肠闭锁患儿手术效果满意.结论:改良肠单层吻合术不仅简便省时,而且有利于吻合口愈合,防止吻合口梗阻发生,尤其适用于小口径肠管.

Objective:To find a way of intestinal anastomosis to reduce the incidence of anastomosis obstruction in the treatment of neonatal intestinal atresia.Methods:The modified singlelayer intestinal anastomosis was performed on and compared with the two-layer intestinal anastomosis in dogs.The some anastomosis was also performed on 7 neonates with intestinal atresia.The anastomosis line of the two methods were observed under microscope.Results:Two weeks after the experiments,the hydroxyproline concentrations(standing for collagen concentration)in the suture lines were 43.527 μg/mg dry weight and 41.423 μg/mg dry weight in the modified single-layer suture group and two-layer suture group respectively.Seven newborns with intestinal atresia were operated by the modified single-layer suture method without stenosis or leakage.Conclusions:The modified single-layer anastomosis is easy,time-saving and preventing early obstruction following the anastomosis of tiny calibre of neonatal intestine
