目的 探讨髂腹股沟皮瓣治疗儿童鞭炮炸伤致虎口重度挛缩的手术疗效.方法 2007至2009年,对9例儿童鞭炮炸伤致虎口重度挛缩的患儿,采用髂腹股沟皮瓣进行治疗.9例均为男性,年龄7~14岁,平均11.4岁.手术分两步,一期行髂腹股沟皮管成形术,皮管远端与第1、2掌骨基底背侧缝合.6周后行断蒂术,同时开大虎口.术后3周拔除弓形针.结果 术后随访时间为6~30个月.9例患儿虎口距离均可达健侧的90%以上,平均可达96%,手的握持功能恢复良好.结论 采用髂腹股沟皮瓣治疗儿童鞭炮炸伤导致虎口重度挛缩是临床首选的简便安全、疗效可靠的手术方式.

목적 탐토가복고구피판치료인동편포작상치호구중도련축적수술료효.방법 2007지2009년,대9례인동편포작상치호구중도련축적환인,채용가복고구피판진행치료.9례균위남성,년령7~14세,평균11.4세.수술분량보,일기행가복고구피관성형술,피관원단여제1、2장골기저배측봉합.6주후행단체술,동시개대호구.술후3주발제궁형침.결과 술후수방시간위6~30개월.9례환인호구거리균가체건측적90%이상,평균가체96%,수적악지공능회복량호.결론 채용가복고구피판치료인동편포작상도치호구중도련축시림상수선적간편안전、료효가고적수술방식.
Oblective To investigate the surgical results of groin flap transfer to treat severe first web contracture in children caused by firework explosion.Methods From 2007 to 2009 nine children with severe first web contracture caused by firewoik explosion were treated with groin flap transfer.All patients were male.Their ages ranged from 7 to 14 years,with an average of 11.4 years.The surgery was carried out in two stages.In the first stage the groin flap was raised and sewed to the dorsum of the first and second metacarpal base with a skin tube pedicle,Six weeks later at the second stage,flap pedicle was divided and the first web widened.The bow-shaped wire to keep the web space open was removed 3 weeks after the second surgery. Results Postoperatively the patients were followed-up for 6 to 30 months.All patients had a first web space as wide as over 90% of the opposite side,being96%on average.Grasp and grip function was good.Conclusion Groin flap transfer is a relatively simple and reliable method to treat severe first web contracture in children.
