目的 研究肾血管性高血压犬中肾动脉狭窄的超声检测方法及其对模型建立的影响.方法 12只beagle犬按双侧肾动脉直径狭窄率分为>80%组、70%~80%组及假手术组,并以双肾双夹法(2k2c)建立肾血管性高血压犬模型.术后1周以彩超检测缩窄犬狭窄处及狭窄远段的肾动脉口径及血流收缩期峰值流速(PSV),并与假手术犬相对照计算肾动脉的狭窄率;在术前1天,术后第2天、4个月、8个月及12个月动态检测犬的尿素氮(BUN)及肌酐(Cr);术后1年比较缩窄犬及假手术犬的肾脏及颈动脉的病理结果.结果 实验早期,连续2只双侧肾动脉狭窄>80%犬在术后4天内因急性肾功能衰竭而死亡,该分组被放弃;5只双侧肾动脉缩窄70% ~80%犬均形成高血压;术后第7天彩超证实:左右两侧肾动脉的狭窄率分别为(73.9±4.2)%及(73.2±4.4)%,这和左右肾动脉狭窄处的PSV与假手术犬左右肾动脉的平均PSV的比率相一致;术后1年,病理切片发现缩窄犬部分肾组织透明样变性,颈动脉有明显斑块形成,BUN和Cr无明显改变.5只假手术犬术后血压无明显升高,术后1年肾组织及颈动脉无明显异常改变.结论 肾动脉缩窄到合适的程度是2k2c法建立肾血管性高血压犬模型的关键性技术;彩超能够准确检测肾血管性高血压犬肾动脉的狭窄程度,为实验做出指导.

Objective To investigate the ultrasonic detection method ant influence on model establishment of renal artery stenosis in renovascular hypertensive canine.Methods Twelve beagles in this experiment were assigned to stenosis rate > 80%,70% ~ 80% and sham-operated group according to the bilateral renal artery diameter stenosis,and induced renovascular hypertension by 2 kidney 2 clip (2k2c) method.One week after operation,the diameter on the stenosis site and the distal segment of renal artery and the corresponding peak systolic velocity (PSV) in constricted beagles were detected by color ultrasound, and compared with sham-operated beagles to calculate the stenosis rate.The serum urea nitrogen (BUN) and creatinine (Cr) on the preoperative 1st day and the postoperative 2nd day,the 4th,the 8th and the 12th month were dynamically measured.One year after operation,the histology findings of kidney and carotid artery were compared between constricted and sham animals.Results In the preliminary stage of the experiment,2 beagles consecutively died of acute renal failure within 4 postoperative days,so this method was given up.All 5 beagles with the bilateral renal artery constriction of between 70% and 80% developed hypertension, 7 days after operation, the stenosis rates of left and right renal artery were respectively revealed by color ultrasound as (73.9 ± 4.2)% and (73.2 ± 4.4)%,which were consistent with the ratios of the PSV on the stenosis site of left and right constricted renal artery and the mean PSV of left and right sham renal artery.One year after operation,histology findings identified transparent degeneration in part renal tissues and obvious carotid plaque formation in constricted beagles,and the BUN and Cr didn' t exhibit significant changes either.None of 5 sham beagles became hypertensive,and kidney and carotid artery histology didn' t show observable abnormal features 1 year after operation.Conclusions An appropriate stenosis degree of renal srtery is the key tenique of 2k2c method in the establishment of renovascular hypertensive canine medel; color ultrasound can reveal the accurate stenosis degree of renal artery in renovascular hypertensive beagles,which may make a guidance for the experiment.
