目的：研究陈旧性心肌梗死患者心脏自主神经昼夜节律改变。方法：应用24h动态心电图系统对正常人及陈旧性心肌梗死患者心率变异性（Heart Rate Variability,HRV）作定量分析，以SDNN(R-R间期)和HF(高频成分)作为HRV指标。结果：正常人SDNN和HF均表现明显的昼夜差异，白昼HRV显著低于夜间（P＜0.05）。陈旧性心肌梗死患者SDNN和HF昼夜差异无显著性，但从24h分布看，HF仍表现昼低夜高的趋势，SDNN则无此变化。结论：陈旧性心肌梗死患者的自主神经昼夜节律改变很可能对其再次发生急性冠脉事件产生影响。

Objective:To study the change of circadian rhythm of cardiac autonomic nervous system (ANS) in old myocardial infarction (OMI) patients. Methods:We measured HRV from standard 24-hour electrocardiogram recordings in OMI patients and normal subjects. Standord deviation of individual normal R-R intervals (SDNN) and high frequency(HF) were used to measure HRV. Results:For normal subjects both SDNN and HF have significant circadian rhythm difference that SDNN and HF are lower during daytime than nightime (P＜0.05). For OMI patients no apparent circadian rhythm occurrs but there is tendency of approximate change to normal subjects for HF measurement. Conclusion:The change of circadian rhythm of cardiac ANS in OMI patients may influence the recurrance of acute coronary accidents.
