目的 探讨Tei指数在评价海员慢性阻塞性肺病患者右心室功能中的作用.方法 在我院2006年至2007年住院的海员患者及健康体检者中选取慢性阻塞性肺病患者38例,正常对照组98例.行超声心动图检查测量右心Tei指数、肺动脉收缩压等.结果 (1)与正常对照组相比,慢性阻塞性肺病患者右心室等容舒张时间(IRT)及等容收缩时间(ICT)明显延长,射血时间(ET)明显缩短,右室Tei指数明显升高(P<0.01);(2)肺动脉高压组间比较显示肺动脉高压的程度对右室Tei指数有一定影响,组间比较差异有统计学意义(P<0.01).(3)右心大小组间比较显示右室扩大的Tei指数增大,但组间比较差异无统计学意义(P>0.05).结论 (1)Tei指数是评价慢阻肺患者右心室收缩舒张功能简便、敏感、综合的多普勒超声新指标;(2)右心室Tei指数受压力负荷增加影响大,受容量负荷增加影响小,其右室功能受损程度与肺动脉收缩压升高有一定相关性.

목적 탐토Tei지수재평개해원만성조새성폐병환자우심실공능중적작용.방법 재아원2006년지2007년주원적해원환자급건강체검자중선취만성조새성폐병환자38례,정상대조조98례.행초성심동도검사측량우심Tei지수、폐동맥수축압등.결과 (1)여정상대조조상비,만성조새성폐병환자우심실등용서장시간(IRT)급등용수축시간(ICT)명현연장,사혈시간(ET)명현축단,우실Tei지수명현승고(P<0.01);(2)폐동맥고압조간비교현시폐동맥고압적정도대우실Tei지수유일정영향,조간비교차이유통계학의의(P<0.01).(3)우심대소조간비교현시우실확대적Tei지수증대,단조간비교차이무통계학의의(P>0.05).결론 (1)Tei지수시평개만조폐환자우심실수축서장공능간편、민감、종합적다보륵초성신지표;(2)우심실Tei지수수압력부하증가영향대,수용량부하증가영향소,기우실공능수손정도여폐동맥수축압승고유일정상관성.
Objective To investigate the evaluation of Tei index on right ventricular function in patients with chronic obstructive pulmonary diseases ( COPD ). Methods Tirty-eight cases with COPD and ninty-eight healthy people were selected as COPD groups and normal control group. Tei index of right ventricular on echocardiography and pulmonary arterial systolic pressure were observed. Results (1) There were significant differeces of isovolumic relaxation time ( IRT) and isovolumetric contraction time ( ICT) , ejection time ( ET) and Tei index of right ventricular between healthy group and normal control group (P < 0.01). (2) In comparison of COPD group, it showed that tei index was changed by the degree of PAH(P< 0.01). There was significant differece in COPD group comparison (P < 0.01). (3) Tei index in right ventricular enlargement increased by PAH while decreased by volume load. Conclusions (1) Tei index is one of convenient, sensitive and general Doppler ultrasound index which can evaluate function of ventricular contraction and diastolic in patients with COPD. (2) Tei index in right ventricular enlargement was influced by pressure load and volume load, respectively. There was a correlation between the degree of right ventricular function damage and increasing of pulmonary arterial systolic pressure.
