目的 探讨解剖型钢板治疗粉碎性尺骨鹰嘴骨折的疗效.方法 2002年2月至2007年7月,共52例移位粉碎性尺骨鹰嘴骨折的患者,男37例,女15例,平均年龄37.6岁.按照Mayo分型:其中ⅡB型31例,ⅢB型21例.所有骨折均行切开复位解剖型钢板内固定.结果 术后49例获得随访,随访时间为12~20个月,平均14.2个月.所有骨折均于3个月内愈合,无伤口感染;3例由于钢板皮下激惹,取出内固定后症状缓解.术后采用Broderg和Money评分标准进行评估:30例ⅡB型骨折中优22例,良6例,可2例,差0例,优良率为93.3%;19例ⅢB型骨折中优6例,良8例,可4例,差1例,优良率为73.7%.两组疗效差别有统计学意义(P<0.05).结论 解剖型钢板是治疗粉碎性尺骨鹰嘴骨折的安全有效的方法,Mayo分型可作为判断骨折预后的依据.

Objective To evaluate the clinical outcomes of congruent plate fixation for treatment of displaced comminuted olecranon fractures. Methods From February 2002 to July 2007, fifty-two patients with displaced comminuted olecranon fractures were treated with open reduction and congruent olecranon plate fixation. There were 37 male patients and 15 female patients, with a mean age of 37. 6 years. According to Mayo classification, there were 31 type Ⅱ B fractures and 21 type Ⅲ B fractures. Results All 49 cases were followed up for an average of 14.2 months (range 12 to 20 months). All fractures were healed within 3 months No wound infection was found in all cases. Three cases experienced symptomatic hardware irritation that was alleviated by hardware removal. The postoperative function was evaluated by Broderg and Morrey score system. In 30 cases of type Ⅱ B fractures, the results were excellent in 22 cases, good in 6 cases, and fair in 2 cases with 93.3% good or excellent rate. In 19 cases of type Ⅲ B fractures, the results were excellent in 6 cases, good in 8 cases, fair in 4 cases and bad in 1 case with 73.7% good or excellent rate. The difference in surgical outcomes between type Ⅱ B and type Ⅲ B fractures was statistically significance( P < 0.05). Conclusion Congruent plate fixation is an effective treatment option for displaced comminuted olecranon fractures with a good functional outcome. The classification system of Mayo can predict the functional outcome of displaced comminuted olecranon fractures.
