目的 探讨血液灌流联合血液透析对尿毒症周围神经病变的疗效和机制.方法 将36例尿毒症周围神经病变的血液透析患者随机分为血液灌/血液透析组(HP-HD)和常规血液透析组(HD).HP-HD组在每周2次常规血液透析的同时,接受每周1次HP-HD治疗;常规血液透析组(HD)行每周3次常规HD治疗.观察、记录治疗前、治疗2个月后两组患者周围神经病变的临床症状、血PTH浓度和正中神经、胫神经、腓总神经的感觉神经传导速度(SCV).结果 经过2个月的治疗后,HD组患者周围神经病变的临床症状、血PTH、感觉神经传导速度无明显改善,而HP-HD组患者周围神经病变的临床症状、血PTH及感觉神经传导速度(SCV)均较治疗前有明显改善,其程度显著优于HD组(P<0.05).结论 血液灌流联合血液透析对尿毒症周围神经病变疗效确切.

목적 탐토혈액관류연합혈액투석대뇨독증주위신경병변적료효화궤제.방법 장36례뇨독증주위신경병변적혈액투석환자수궤분위혈액관/혈액투석조(HP-HD)화상규혈액투석조(HD).HP-HD조재매주2차상규혈액투석적동시,접수매주1차HP-HD치료;상규혈액투석조(HD)행매주3차상규HD치료.관찰、기록치료전、치료2개월후량조환자주위신경병변적림상증상、혈PTH농도화정중신경、경신경、비총신경적감각신경전도속도(SCV).결과 경과2개월적치료후,HD조환자주위신경병변적림상증상、혈PTH、감각신경전도속도무명현개선,이HP-HD조환자주위신경병변적림상증상、혈PTH급감각신경전도속도(SCV)균교치료전유명현개선,기정도현저우우HD조(P<0.05).결론 혈액관류연합혈액투석대뇨독증주위신경병변료효학절.
Objective To observe the efficacy and mechanism of the hemoperfusion-hemodialysis in treating uremic per-pheral neuropathy. Methods Thirty-six patients with uremic peripheral neuropathy were assigned randomly to hemoperfusion-hemodialysis group,hemodialysis group. Patients of hemoperfusion-hemodialysis group received regular hemodialysis twice a week and hemoparfusion-hemodialysis once a week. The hemodialysis group received regular hemodialysis thrice a week. The clinical symptoms, PTH, the nerve reflex and the sensory conduction velocity(scv) of median nerve, tibial nerve and lateral popliteal nerve were observed in each group before,and 2 months cost treatment respectively. Results After treatment, the clinical symptoms, PTH, the nerve reflex and the scv showed no improvement in the hemodialysis group, but significantly improvement in the he-moperfusion-hemodialysis group(P<0.05). Conclusion Hemodiafiltration are effective for patients with uremic peripheral heuropathy.
