目的 研究生理盐水气道湿化对老年患者全麻术后排痰的影响.方法 将58例全麻手术后的老年患者,随机分为常规组(n=29)及湿化组(n=29),常规组按常规进行药物雾化吸入,间歇期使用鼻导管或面罩吸氧,湿化组在常规药物雾化吸入的间歇期持续使用生理盐水湿化,观察患者咯出第1口痰时间、术后痰液性状改变的时间、痰量、心率、呼吸及血氧饱和度.结果 与常规组比较,湿化组的排痰时间、痰液性状改变时间缩短(P<0.05),术后前2 d痰量明显增加(P<0.05).结论 老年患者术后在雾化吸入的间歇期持续生理盐水气道湿化,能促进早期排痰,无显著副作用,安全有效,值得临床推广.

목적 연구생리염수기도습화대노년환자전마술후배담적영향.방법 장58례전마수술후적노년환자,수궤분위상규조(n=29)급습화조(n=29),상규조안상규진행약물무화흡입,간헐기사용비도관혹면조흡양,습화조재상규약물무화흡입적간헐기지속사용생리염수습화,관찰환자각출제1구담시간、술후담액성상개변적시간、담량、심솔、호흡급혈양포화도.결과 여상규조비교,습화조적배담시간、담액성상개변시간축단(P<0.05),술후전2 d담량명현증가(P<0.05).결론 노년환자술후재무화흡입적간헐기지속생리염수기도습화,능촉진조기배담,무현저부작용,안전유효,치득림상추엄.
Objective To investigate the effect of saline airway humidification on sputum excretion after operation in elder patients. Methods Elder patients who received operations were randomly divided into routine treatment group with common medicine aerosol inhalation by mask oxygen inhalation, and humidification group with continuous saline airway humidification in intermission of common medicine aerosol inhalation. We mainly examined the time of first sputum, the time of sputum changing characters, sputum amount, heart rate,respiration and blood oxygen saturation. Results Compared with routine group, the humidification group obviously shorten sputum excretion duration, character changing time, and sputum amount significantly increase in 48 hours after operation( P < 0.05 ). Conclusions The continuous saline humidification in intermission of common medicine aerosol inhalation after operations could accelerate the process of sputum excretion. It is efficacy and safety, and it deserves clinical application.
