目的 评价保留植物神经的直肠癌扩大根治术,对减少患者术后泌尿和性功能障碍的作用. 方法 对59例DukesA、B、C期直肠癌患者行保留植物神经的直肠癌扩大根治术,另外设对照组34例行不保留植物神经手术. 结果 泌尿功能障碍:保留组13.6%(8/59),不保留组67.7%(23/34)(P<0.01).男性性功能障碍:保留组勃起障碍22.6%(7/31),射精障碍29.0%(9/31);不保留组分别为60.87%(14/23)和87.0%(20/23),两组比较(P<0.05). 结论 保留植物神经的直肠癌扩大根治手术能够有效地减少患者术后性功能和泌尿功能障碍.

Objective To evaluate autonomic-nerve-preserving extended operation (ANPEO) for rectal cancer in the protection of post-operative urinary and male sexual functions. Methods 59 Dukes A,B and C cases underwent ANPEO for rectal cancer. A group of 34 patients undergoing traditional extended radical operation were selected as controls without preservation of the autonomic nerve. Results Urination disorders: 13.56%(8/59) in the ANPEO group and 67.65%(23/34) in the non-ANPEO group (P<0.01). Male sexual function disorders: 22.58%(7/31)in the ANPEO group of erectile dysfunction, 86.96%(20/23) in the ANPEO group of ejaculatory disorder, 29.03%(9/31) in the non-ANPEO group of erectile dysfunction, 60.87%(14/23) in the non-ANPEO group of ejaculatory disorder.Comparison of the two groups showed (P<0.01). Conclusions ANPEO is effective to reduce postoperative urinary and sexual dysfunctions while maintaining a radical cure for the cancer.
