目的 观察大鼠肺动脉阻断不同时段肺功能及结构的变化及内皮素-1受体拮抗剂的肺保护作用.方法 Wistar大鼠24只,雌雄不分,随机分成4组(每组6只),假手术组(Ⅰ组)、左肺动脉阻断45 min(Ⅱ组)、左肺动脉阻断60 win(Ⅲ组)、左肺动脉阻断60 min前静脉注射内皮素-1受体拮抗剂BQ-123,1 mg/kg(Ⅳ组).实验结束时,检测肺组织的湿/干比(W/D)、血浆内皮素含量(ET-1)、平均肺动脉压力(mPAP)、PaO2、免疫组织化学方法测定肺组织Fas蛋白表达的变化,同时观察肺组织光学显微镜下的变化.结果 与Ⅰ组比较Ⅱ组肺组织的W/D、ET-1、mPAP、PaO2、肺组织Fas蛋白表达变化差异无统计学意义(P>0.05)、Ⅲ组W/D、ET-1、mPAP、Fas蛋白表达增加,PaO<<2下降(P<0.01).Ⅳ组与Ⅲ组比较,W/D、ET-1、mPAP、Fas蛋白表达降低,PaO2升高(P<0.01).光学显微镜下观察见Ⅱ、Ⅳ组肺组织损伤明显轻于Ⅲ组.结论 大鼠可以耐受左侧肺动脉阻断45 min,恢复灌注后缺血-再灌注损伤对肺结构和功能影响较小,而阻断60 min,再灌注后对肺结构和功能均有较明显损伤,内皮素-1受体拮抗剂具有一定的对肺缺血-再灌注损伤的肺保护作用.

목적 관찰대서폐동맥조단불동시단폐공능급결구적변화급내피소-1수체길항제적폐보호작용.방법 Wistar대서24지,자웅불분,수궤분성4조(매조6지),가수술조(Ⅰ조)、좌폐동맥조단45 min(Ⅱ조)、좌폐동맥조단60 win(Ⅲ조)、좌폐동맥조단60 min전정맥주사내피소-1수체길항제BQ-123,1 mg/kg(Ⅳ조).실험결속시,검측폐조직적습/간비(W/D)、혈장내피소함량(ET-1)、평균폐동맥압력(mPAP)、PaO2、면역조직화학방법측정폐조직Fas단백표체적변화,동시관찰폐조직광학현미경하적변화.결과 여Ⅰ조비교Ⅱ조폐조직적W/D、ET-1、mPAP、PaO2、폐조직Fas단백표체변화차이무통계학의의(P>0.05)、Ⅲ조W/D、ET-1、mPAP、Fas단백표체증가,PaO<<2하강(P<0.01).Ⅳ조여Ⅲ조비교,W/D、ET-1、mPAP、Fas단백표체강저,PaO2승고(P<0.01).광학현미경하관찰견Ⅱ、Ⅳ조폐조직손상명현경우Ⅲ조.결론 대서가이내수좌측폐동맥조단45 min,회복관주후결혈-재관주손상대폐결구화공능영향교소,이조단60 min,재관주후대폐결구화공능균유교명현손상,내피소-1수체길항제구유일정적대폐결혈-재관주손상적폐보호작용.
Objective To discuss the lung function and structure changes in different periods of occluding pulmonary artery,and the protective effects of endothelin-1 receptor antagonist against lung ischemia-reperfusion injury in rats.Methods Twenty-four Wistar rats were randomly divided into 4 groups : group Ⅰ ,sham operated:group Ⅱ ,occluding left pulmonary artery for 45 min: group Ⅲ ,occluding left pulmonary artery for 60 min:group Ⅳ ,BQ-123 (1 mg/kg) was given by intravenous dropping before occluding left pulmonary artery for 60 min.The wet to dry weight ratio of lung tissue (W/D) ,the mean pulmonary artery pressure (mPAP),arterial oxygen tension (PaO2) and plasma endothelin-1 (ET-1) were measured respectively in different groups, the expression of Fas protein in lung tissues at the ischemiareperfusion injury area was detected by SABC immunohistochemical technique, and pathological changes were also observed.Results Between groups Ⅰ and Ⅱ,Ⅳ, W/D, mPAP, ET-1 and the PaO2 had no significant difference (P 0.05).The marked increase of W/D,mPAP,ET-1 ,and the decrease of PaO2 ,together with the higher expression of Fas protein were observed in group Ⅲ (P < 0.05).Compared with group Ⅲ, W/D, mPAP, ET-1, and the expression of Fas protein were decreased, while PaO2 was increased in group Ⅳ.The inury of lung tissues was significantly alleviated in in groups Ⅱ and Ⅳ than in group Ⅲ under the light microscopy (P < 0.05).Conclusion Rats are well tolerated 45 min of occluding left pulmonary artery, and the ischemia-reperfusion injury of the lung is significant after 60 min of occluding left pulmonary artery.ET-1 receptor antagonist can protect the lung from isehemia-reperfusion injury to some extent.
