目的探讨"延肾胶囊"对残肾肾小球系膜细胞凋亡及其细胞外基质代谢的影响.方法Wistar大鼠120只,随机分为6组,A组为假手术对照组,B组为5/6肾切除;C,D和E组分别为肾切除+低、中、高剂量延肾胶囊;F组为5/6肾切除+肾衰宁.术后A,B两组给予等容积生理盐水灌胃.定量喂养,自由饮水.于术后15,90天测定各指标,每组每时相点观察10只大鼠.结果术后B组血浆血管紧张素Ⅱ浓度、肾小球系膜细胞增殖指数和凋亡指数、肾组织TGF-β1,CoⅣ,Fas,FasL积分光密度等指标显著高于A组.C,D和E组血浆血管紧张素Ⅱ浓度、肾小球系膜细胞增殖指数、肾组织TGF-β1和CoⅣ积分光密度高于A组,但显著低于B组,随着延肾胶囊剂量增加,上述指标有逐渐降低的趋势;肾小球系膜细胞凋亡指数、Fas和FasL积分光密度等指标显著高于A,B两组,随着延肾胶囊剂量增加,上述指标有逐渐升高趋势.F组血浆血管紧张素Ⅱ浓度、肾小球系膜细胞增殖指数、肾组织TGF-β1和CoⅣ积分光密度,低于B组,但高于D,E两组;肾组织Fas和FasL积分光密度等指标显著高于A和B组,稍低于D、E两组,肾小球系膜细胞凋亡指数与B组无显著差异,显著低于C,D和E3组.结论"延肾胶囊"能显著抑制5/6肾切除大鼠残肾血管紧张素Ⅱ和TGF-β1的高表达,增加Fas和FasL的表达,抑制肾小球系膜细胞过度增生,促进增生的系膜细胞凋亡,稳定残肾小球系细胞数量,减轻细胞外基质的堆积,呈剂量依赖性;"肾衰宁"能抑制系膜细胞增生,但不能促使增生的系膜细胞凋亡.

Objective: To elucidate the effect of Yan-Shen capsule on glomerular mesangial cell apoptosis and extracellular matrix metabolism of remnant kidney. Methods: 120 adult Wistar rate were randomly divided into 6 groups, 20 rats in each group. Rats of group A were sham-operatied, rats of group B, C, D, E and drug Shen-Shuai-Ning capsule. Group A and B were given same volume of saline. The rats were fed with standard big granule feed for 15 g/ 100 g bw/d. and permitted to drink water freely. On the 15 and 90 after operation, plasma angiotensin Ⅱ (Ang Ⅱ ) concentration, glomerular mesangial cell proliferation and apoptosis index, renal TGF-β1, collegen Ⅳ(CoW), Fas and Fas ligand (FasL) expression integra of optical density (IOD)were observed. There were 10 rats for each group and each time point. Results: It was found that, after operation, AngⅡ concentration, Glomerular mesangial cell proliferation and apoptosis index, and IOD and TGF[β1, CoⅣ, Fas and FasL in group B were obviously higher than group P (P<0.05). In group C, D and E,AngⅡ concentration, glomerular mesangial cell proliferation index, and IOD of TGF-β1 and CoⅣ were higher than that of group A, but much lower than that of group B (P <0.05), the higher the dosage of Yan-Shen capsule, the higher the above markers. Ang Ⅱ concentration, glomerular mesangial cell proliferation index, and IOD of TGF-β1 and CoⅣ in group F were lower than that of group B (P<0.05), the higher than that of group D and E. The IOD of Fas and FasL in group F was higher than that of group A and B (P<0.05), but was lower than that of group D and E. The glomerular mesangial cell apoptosis index in group F was similar to that of group B, but much lower than that of group C, D and E (P <0.05). Conclusions: There are high expression of Ang Ⅱ, TGF-β1 Fas an dFasL in remnant kidney of 5/6 nephrectomized rats, which resulted in the obvious proliferation and slightly higher apoptosis of glomerular mesangial cell and accumulation of extracrellular matrix. Yan-Shen capsule could suppress the expression of Ang Ⅱ and TGF-β1, facilitate the expression of Fas and FasL, as a result, suppress proliferation, facilitate apoptosis and stabilize quantity of the glomerular mesangial cell, and relieve the accumulation of extracellular matrix in dosage dependent manner.Shen-Shuai-Ning capsule could suppress proliferation but could not fecilitate apoptosis of glomerular mesangial cell.
