目的 观察重组人生长激素(rhGH)对急性呼吸窘迫综合征(ARDS)机械通气患者营养状况和免疫功能的影响.方法 在ICU病房住院的ARDS机械通气患者60例,随机分为治疗组30例和对照组30例.两组患者给予相同方案的营养支持治疗,治疗组加用皮下注射rhGH 8 u/d,共7d.检测两组患者治疗前1d、治疗后4d、7d的各营养指标和免疫指标的变化.结果 两组患者治疗前营养状况和细胞免疫指标均低于正常.治疗后4d治疗组患者转铁蛋白(TFN)增加明显(P＜0.05),前白蛋白(PA)增加显著(P＜0.05)；CD3、CD4、CD8、CD4/CD8比值均增加明显(P＜0.05).治疗后7d,治疗组患者TFN、PA、CD3 、CD4、CD8、CD4/CD8比值均较对照组增加显著(P＜0.01).结论 小剂量生长激素联合营养支持治疗能改善急性呼吸窘迫综合征机械通气患者的营养状况,同时能提高ARDS患者的免疫功能,提高患者抢救的成功率.

목적 관찰중조인생장격소(rhGH)대급성호흡군박종합정(ARDS)궤계통기환자영양상황화면역공능적영향.방법 재ICU병방주원적ARDS궤계통기환자60례,수궤분위치료조30례화대조조30례.량조환자급여상동방안적영양지지치료,치료조가용피하주사rhGH 8 u/d,공7d.검측량조환자치료전1d、치료후4d、7d적각영양지표화면역지표적변화.결과 량조환자치료전영양상황화세포면역지표균저우정상.치료후4d치료조환자전철단백(TFN)증가명현(P＜0.05),전백단백(PA)증가현저(P＜0.05)；CD3、CD4、CD8、CD4/CD8비치균증가명현(P＜0.05).치료후7d,치료조환자TFN、PA、CD3 、CD4、CD8、CD4/CD8비치균교대조조증가현저(P＜0.01).결론 소제량생장격소연합영양지지치료능개선급성호흡군박종합정궤계통기환자적영양상황,동시능제고ARDS환자적면역공능,제고환자창구적성공솔.
Objective To observe the influence of the recombinant human growth hormone (rhGH) on nutritional status and immune function in patients with acute respiratory distress syndrome(ARDS).Methods 60 ARDS patients with mechanical ventilation hospitalized in the ICU ward were randomly divided into the treatment group and the control group,each group had 30 cases.The patients of the two groups were given the same treatment program of nutritional support,and the patients of the treatment group were administered with rhGH 8u/d,subcutaneous injection,treatment course was 7d.1d before treatment,4d,7d after treatment,the nutrition indicators and changes in immune parameters of the two groups were detected.Results The nutritional status before treatment and the cellular immune parameters of the two groups were lower than normal.4d after treatment,the transferrin requirements(TFN),prealbumin(PA),CD3,CD4,CD8 and CD4/CD8 ratio of the treatment group increased significantly( all P ＜0.05 ) ;7d after treatment,TFN,PA,CD3,CD4,CD8,CD4/CD8 ratio of the treatment group increased significantly compared with the control group ( P ＜ 0.01 ).Conclusion Low-dose hormone therapy combined with the nutritional support could improve the nutritional status,the immune function of ARDS patients with mechanical ventilation and enhance the success rate of patients in emergency.
