目的 评价便携式超声仪在海战颅脑伤手术中的应用价值。方法 急诊手术病例6例,包括血肿清除术5例,内镜颅内子弹取出术1例。所有手术均在实时超声辅助下进行。结果 术中超声检查均与术前头颅CT结果一致。所有病变均在超声辅助下精确定位,并完整切除。无症状恶化及死亡者,无严重手术并发症发生。结论 术中实时超声应用于急诊脑内血肿清除和颅内异物取出术,能够更好地定位病变部位,决定皮层的切口和手术路径,动态了解病灶情况,判断病变的切除范围,避免损伤重要血管,在海战颅脑伤救治中具有良好的应用前景。
目的 評價便攜式超聲儀在海戰顱腦傷手術中的應用價值。方法 急診手術病例6例,包括血腫清除術5例,內鏡顱內子彈取齣術1例。所有手術均在實時超聲輔助下進行。結果 術中超聲檢查均與術前頭顱CT結果一緻。所有病變均在超聲輔助下精確定位,併完整切除。無癥狀噁化及死亡者,無嚴重手術併髮癥髮生。結論 術中實時超聲應用于急診腦內血腫清除和顱內異物取齣術,能夠更好地定位病變部位,決定皮層的切口和手術路徑,動態瞭解病竈情況,判斷病變的切除範圍,避免損傷重要血管,在海戰顱腦傷救治中具有良好的應用前景。
목적 평개편휴식초성의재해전로뇌상수술중적응용개치。방법 급진수술병례6례,포괄혈종청제술5례,내경로내자탄취출술1례。소유수술균재실시초성보조하진행。결과 술중초성검사균여술전두로CT결과일치。소유병변균재초성보조하정학정위,병완정절제。무증상악화급사망자,무엄중수술병발증발생。결론 술중실시초성응용우급진뇌내혈종청제화로내이물취출술,능구경호지정위병변부위,결정피층적절구화수술로경,동태료해병조정황,판단병변적절제범위,피면손상중요혈관,재해전로뇌상구치중구유량호적응용전경。
Objective To evaluate the clinical value of portable ultrasonography in craniocerebral injury sustained in sea battle. Methods There were 6 emergency surgical cases, 5 cases of hematoma clearing, 1 case of intracranial bullet removal, which were all operated with the aid of portable ultrasonography. Results The images of ultrasonography during the operation were identical to those obtained from preoperative CT scans. All the pathological lesions could accurately be located with ultrasonography and were completely removed. There were neither cases of deterioration and dead, nor cases of complications. Conclusions Realtime application of ultrasonography in the process of emergency craniocerebral hematoma clearing and foreign object removal had the following advantages: more accurate location of pathological lesiobs; optimal determination of cortical incision and surgical approach; full knowledge of the foci ; better judgement on the extent of excision; and avoidance of injury to important blood vessels. It seemed that there was a good prospect for ultrasonography to be used in emergency medical care of craniocerebral injury sustained in sea battle.
