目的探讨超声造影在肝脾外伤非手术治疗中的作用.方法 48例血流动力学稳定的肝脾外伤患者行非手术治疗,对其进行血流动力学及常规超声与超声造影结合监测随访.结果 44例非手术治疗成功,3例中转手术,1例死亡.超声造影发现肝外伤性动脉瘤各1例.非手术治疗成功组血流动力学持续稳定,超声造影低-无增强区逐渐缩小.中转手术组血流动力学不稳定,超声发现腹水进行性增加.超声造影显示实质内无增强区范围增大.结论血流动力学变化情况在肝脾外伤非手术治疗中起决定性作用,超声造影结合常规超声对肝脾外伤非手术治疗进行监测和随访,对于发现并发症、病情变化以及预后有一定辅助作用.

Objective To explore the value of contrast-enhanced ultrasound(CEUS) in the nonoperative treatement of hepatic or splenic trauma.Methods Forty-eight stable hemodynamics patients with hepatic or splenic trauma were treated nonoperatively.And they were all monitored with hemodynamics and ultrasonography (combined base ultrasonography with CEUS).Results Forty-four cases (91.7%)were cured nonoperatively.Three cases underwent operation later.One case died.Traumatic aneurysm of liver and spleen was found by CEUS.The hemodynamics is stable and the hematoma in CEUS grows smaller in the nonoperative group,whereas the operative group is opposite.Conclusions The change of hemodynamics is decisive in the treatment of hepatic or splenic trauma.CEUS combined with base ultrasound is helpful in monitoring and the follow-up of nonoperative treatment.
