目的 探讨非同轴光照明下显微镜巩膜扣带术的可行性及效果.方法 回顾分析了137例(137只眼)孔源性视网膜脱离患者,在显微镜下采用非同轴光照明进行巩膜扣带术,手术后随访6~13个月.结果 130只眼手术全程均在显微镜下完成,手术过程顺利,7只眼术中改用倒像镜进行手术.视网膜一次性完全复位123只眼.4只眼再次采用外加压后复位;3只眼因术后严重PVR接受玻璃体手术.12只眼手术后视网膜下片状出血,2个月内均吸收.手术后3个月视力提高73只眼;不变40只眼;下降17只眼.结论 非同轴光照明下显微镜巩膜扣带术具有手术创伤小,术野清晰,手术操作简便等特点,在临床上具有实用价值.

Objective To discuss the feasibility and effects of scleral buckling (SC) under microscope with non-coaxial optical illumination. Methods Retrospected and analyzed 137 cases (137 eyes) with rhegmatogenous retinal detachment. The procedure of scleral buckling was performed with the operating microscope with non-coaxial optical illumination. All patients were followed up 6 to 13 months. Results Indirect ophthalmoscopy reselected for the dioptric media opacity was applied in the operation of 7 cases (7 eyes) underwent SC, the rest cases used microscope with non-coaxial optical illumination throughly underwent SC.Retinal was reattached in 123 of 130 eyes, retinal detachment recurred in 7 eyes, 4 eyes underwent SC again and the retinal was reattached, 3 eyes underwent PPV for the severe PVR after the first surgery. Subretinal hemorrhage occurred in 12 eyes and was absorbed in 2 months. The visual acuity was improved in 73 eyes, unchanged in 40 eyes and decreased in 17 eyes. Conclusions The benefit ofscleral buckling under microscope with non-coaxial optical illumination includes minimally invasion, convenience, direct viewing. The method has great applied value in clinic.
