目的 探讨洋参二醇皂苷注射液(IPQDS)对急性心肌梗死犬心肌氧代谢及冠脉循环的影响.方法 麻醉开胸结扎犬左冠状动脉前降支6 h建立急性心肌梗死模型,同时静脉滴注 IPQDS 5、10、20 mg/kg 进行治疗,以CORNIHG 178型血气分析仪测定动、静脉血氧含量,观察心肌氧代谢变化,通过MP-27型电磁流量计记录冠状动脉血流量及冠脉阻力,观察冠脉循环情况.结果 与梗死模型组比较,静脉滴注 IPQDS 10、20 mg/kg,可使急性心肌梗死 6 h犬心肌氧利用率、心肌耗氧指数及心肌耗氧量均明显降低(P<0.05或P<0.01),心肌血流量明显增加,冠脉血管阻力明显降低(P<0.05或P<0.01),IPQDS 5 mg/kg对上述各参数均无明显影响(P>0.05).结论 IPQDS可通过改善冠脉循环增加心肌血液供给,同时降低心肌耗氧及降低心肌氧利用率发挥抗急性心肌缺血作用.

Objective To approach the effects of injection of Panax quinquefolium diolsaponins(IPQDS) on the myocardial oxygen metabolism and the coronary circulation in dogs with acute myocardial infarction(AMI). Methods The AMI model was induced by myocardial infarction for 6 h in dogs of left anterior descending coronary occulusion. After 90 min of treatment by IPQDS, the blood oxygen of artery and vein were determined by analysis instrument of blood gas with CORNIHG 178 type. At the same time, the blood flow of coronary artery was determined by electromagnetism flowmeter with MP-27 type. Results After treated by IPQDS (in a dosage of 10 and 20 mg/kg iv drip for 90 min), the cardiac oxygen consumption, cardiac oxygen consumption index and myocardial oxygen utilization rate were decreased. Meanwhile the myocardial blood flow (MBF) was increased and coronary vascular resistance (CVR) was decreased significantly but IPQDS (in a dosage of 5 mg/kg iv drip for 90 min) had no significant effect on above-mentioned all parameters. Conclusions IPQDS has protective effects on AMI including improving the coronary circulation, increasing myocardial blood supply and decreasing cardiac oxygen consumption and myocardial oxygen utilization rate.
