目的探讨心理干预对老年甲状腺功能减退症(甲减)抑郁患者临床康复的影响.方法 42例老年甲减后抑郁患者按随机数字表法分为2组,每组21例.2组患者均接受常规抗甲减药物、抗抑郁及康复治疗,心理干预组同时进行心理干预,分别进行汉密顿抑郁量表(HAMD)、神经功能缺损评分(NFA)、Barthel生活能力指数(BI)及运动功能评分(FMA)测评.结果 2组治疗前各项评分差异无显著性(P＞0.05).治疗4周后对照组HAMD评分为[(15.5±3.5)分]、NFA为(15.0±6.3)分、BI为(38.5±20.6)分、FMA为(43.9±21.2)分;心理干预组HAMD评分(10.2±3.2)分、NFA为(11.6±5.9)分、BI为(50.2±21.8)分、FMA为(56.6±20.6)分,2组差异具有显著性(P＜0.05).结论心理干预能有效缓解老年甲减后患者的抑郁情绪,提高康复效果.
목적 탐토심리간예대노년갑상선공능감퇴증(갑감)억욱환자림상강복적영향.방법 42례노년갑감후억욱환자안수궤수자표법분위2조,매조21례.2조환자균접수상규항갑감약물、항억욱급강복치료,심리간예조동시진행심리간예,분별진행한밀돈억욱량표(HAMD)、신경공능결손평분(NFA)、Barthel생활능력지수(BI)급운동공능평분(FMA)측평.결과 2조치료전각항평분차이무현저성(P＞0.05).치료4주후대조조HAMD평분위[(15.5±3.5)분]、NFA위(15.0±6.3)분、BI위(38.5±20.6)분、FMA위(43.9±21.2)분;심리간예조HAMD평분(10.2±3.2)분、NFA위(11.6±5.9)분、BI위(50.2±21.8)분、FMA위(56.6±20.6)분,2조차이구유현저성(P＜0.05).결론 심리간예능유효완해노년갑감후환자적억욱정서,제고강복효과.
Objective To evaluate the efficiency of psychological interventions on aged depressive patients with post-hypothyroidism.Methods 42 patients were investigated by using case-control study.21 of them were recruited as the experimental group and control group.Two groups received anti-hypothyroidism & anti-depressant medications and rehabilitation treatment.At the meantime the experimental group received psychological interventions.Before and after treatment,2 groups were evaluated by Hamilton Depression Rating Scales(HAMD),Fugl-Meyer Assessment(FMA),Neurological Function Deficits Assessment (NFA),Barthel (Barthel Index,BI).Results Before intervention,the score of HAMD,NFA,BI,FMA was (15.5±3.5),(15.0±6.3),(38.5±20.6),(43.9±21.2) respectively in control group.After intervention,the score of HAMD,NFA,BI,FMA was (IO.2±3.2),(11.6±5.9),(50.2±21.8),(56.6±20.6) respectively in control group.Compared with control group ,scores of the scales were significant difference between two groups after 4 weeks psychological interventions (P ＜ 0.05).Conclusion The psychological interventions are effective in improving activities of daily living on aged depressive patients with post-hypothyroidism.
