目的探讨老年脑梗死患者肺炎衣原体(Gpn)感染与血脂的关系.方法采用微量免疫荧光法检测老年脑梗死组88例及对照组80例外周血CpnlgG抗体,同时测定血脂.结果脑梗死组及对照组血清CpnlgG抗体阳性率分别为79.5%及47.8%,差异有统计学意义(P<0.01),脑梗死组血脂总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆同醇(LDL-C)、载脂蛋白A(apoA)显著高于对照组(P<0.01),而高密度脂蛋白胆固醇(HDL-C)脑梗死组显著低于对照组(P<0.01);Cpn感染者特异性抗体IgG滴度与血中TG、LDL-C及apoA有明显相关性(r分别为0.47、0.65和0.43),而与HDL-C成负相关(r=0.81)(P<0.01).结论 Cpn感染可能是老年患者脑梗死的一个重要发病机制之一,Cpn可能通过影响脂质代谢在脑梗死的发生、发展中起作用.
목적 탐토노년뇌경사환자폐염의원체(Gpn)감염여혈지적관계.방법 채용미량면역형광법검측노년뇌경사조88례급대조조80예외주혈CpnlgG항체,동시측정혈지.결과 뇌경사조급대조조혈청CpnlgG항체양성솔분별위79.5%급47.8%,차이유통계학의의(P<0.01),뇌경사조혈지총담고순(TC)、감유삼지(TG)、저밀도지단백담동순(LDL-C)、재지단백A(apoA)현저고우대조조(P<0.01),이고밀도지단백담고순(HDL-C)뇌경사조현저저우대조조(P<0.01);Cpn감염자특이성항체IgG적도여혈중TG、LDL-C급apoA유명현상관성(r분별위0.47、0.65화0.43),이여HDL-C성부상관(r=0.81)(P<0.01).결론 Cpn감염가능시노년환자뇌경사적일개중요발병궤제지일,Cpn가능통과영향지질대사재뇌경사적발생、발전중기작용.
Objective To investigate the relationship between Chlamydia pneumoniae (Cpn)infection in infartion of elderly patients. Methods The serums peeific antibodies to CpnIgG were measured by using microimmunofluoreseence technique in 80 healthy subjects as control group and 88 elderly infarction patients. The levels of blood-fat were examined. Results The positive rate of Cpn specific IgG antibody was 79.5％ in infarction group,47.8％ in control group, the patients with infarction had significantly higher positive rate of CpnIgG than the subjects in control group(P<0.01). Total cholesterol, triglycerides(TG) ,low density lipoprotein cholesterol(LDL-C), apolipoproteinA (apoA), pdipoproteinB (apoB) in infarction group were higher than those in control group(P<0.01) ,while high density lipoprotein cholesterol(HDL-C)in infarction group was significantly lower than that in control group(P<0.01). Cpn IgG is related positively to TG,LDL-C and apoA(r=0.47, 0.65,0.43) ,but negatively to HDL-C in infarction group (r=-0.81)(P<0.01). Conclusion Cpn infection is also an important risk factor for ischemic stroke, particularly in elderly patients, Cpn infection may play a role in the pathogenesis of infarction through lipid metabolic disturbance.
