背景:感音神经性聋的脑干听觉诱发电位(brainstem auditory evokedpotential,BAEP)波型分化差,对感音神经性聋做出进一步诊断较困难,应用疏波短声给声可提高感音性聋的BAEP出波率和改善波形分化.目的:研究应用疏波短声给声刺激提高感音性聋的BAEP出波率和改善波形分化的现象.设计:前后对照试验.地点和对象:实验地点:菏泽医学专科学校.1999/2000年间菏泽市立医院耳鼻喉科接诊的耳聋患者中经临床及纯音测听确定为感音神经性聋患者48例、70耳,年龄90 d-42岁,平均10.7岁.干预:检测耳给短声刺激,对侧耳给噪声掩蔽;分别用疏密交替波和疏波短声给声刺激进行BAEP测试,比较两种给声测试的结果.主要观察指标:各波出波率及波形.结果:用疏密交替波短声给声,BAEP各波出波率很低,70耳中出现Ⅰ波16例,出现Ⅱ,Ⅲ,Ⅳ,Ⅴ波各2例,余均呈无反应型.改用单一的疏波短声后,各波出波率明显提高,尤其前3个波更高,70耳中,各波出现例数分别为70,68,52,12和16,各波出现率与前者相比,除Ⅳ波外,其余各波差异均有显著性意义(P<0.05,P<0.01),且波形分化也有改善,最为明显的是Ⅰ波变得更大且呈多峰.结论:应用疏波短声给声有助于BAEP为感音性聋进一步做出定位诊断.

BACKGROUND:Since the wave type of brainstem auditory evoked potential (BAEP) in sensorineural deafness is too difficult to be differentiated from others, it is not an ideal method for differentiating diagnosis. Rarefaction clicks can help improve the wave expressing rate and wave type differentiation of BAEP in sensorineural deafness.OBJECTIVE: To investigate the wave expressing rate and wave type differentiation of BAEP after rarefaction click stimulation in patients with sensorineuraldeafness.DESIGN: Case control study.SETTING and PARTICTPANTS:This study was carried out in Heze Medical College. Seventy ears of 48 patients, aged from 90 days to 42 years old, with the mean age of 10. 7 years old, wereconfirmed as sensorineural deafness by clinical and pure sound examination at the Department of Otorhinolaryngology, Heze Municipal Hospital between 1999 and 2000.INTERVENTIONS: Click stimulation was given to the experimental ear with the opposite side masked by noise; BAEPs were induced by rarefaction-condensation alternating click stimulation and rarefaction click stimulation respectively to compare the differences between them.MAIN OUTCOME MEASURES: Wave expressing rate and wave type.RESULTS: The wave-expressing rate of BAEP was very low if the patients were subjected to rarefaction-condensation alternating clicks. Wave Ⅰ appeared in 16 out of the 70 ears, and waves Ⅱ, Ⅲ, Ⅳ and V appeared in 2ears, respectively, while the other ears had no response. However,wave-expressing rate was markedly increased in these waves induced bysingle rarefaction click stimulation, especially in the first three waves. The number of wave expressing cases was 70, 68, 52, 12 and 16 out of the 70ears, respectively, which was significantly different from the corresponding waves of rarefaction-condensation alternating click group except wave Ⅳ ( P<0.05, P <0.01 ). Moreover wave type was improved, particularly presented by wave Ⅰ enlarged with more peaks.CONCLUSION: Rarefaction click stimulation is beneficial for BAEP in level diagnosis of sensoyineural deafness.
