目的 探讨直肠癌低位前切除术并发吻合口漏的诊断及处理方法. 方法 回顾性分析直肠癌低位前切除术并发吻合口漏16例的临床资料. 结果 186例直肠癌低位前切除术后并发吻合口漏16例,发生率为8.6%,14例经局部积极冲洗引流后治愈,1例发生腹膜炎,急诊行上段结肠造口后治愈;1例高龄患者并发感染性休克死亡. 结论 吻合口漏是直肠癌低位前切除术后最严重的并发症之一,围术期及时有效治疗可降低吻合口漏的发生率.

목적 탐토직장암저위전절제술병발문합구루적진단급처리방법. 방법 회고성분석직장암저위전절제술병발문합구루16례적림상자료. 결과 186례직장암저위전절제술후병발문합구루16례,발생솔위8.6%,14례경국부적겁충세인류후치유,1례발생복막염,급진행상단결장조구후치유;1례고령환자병발감염성휴극사망. 결론 문합구루시직장암저위전절제술후최엄중적병발증지일,위술기급시유효치료가강저문합구루적발생솔.
Objectiye To explore the diagnostic and therapeutic methods of anastomotic leakage after low anterior resection of rectal cancer. Methods The clinical data of 16 cases with anastomotic leakage after sphincter saving operation which was performed through anterior resection for patients with lower rectal cancer were retrospectively analyzed. Results Among the 186 cases of low anterior resection of rectal cancer,16 cases occurred anastomotic leakage,incidence rate was 8.6% ;14 cases healed through local douche and drainage; 1 case healed by emergency operation and upper colostomy after bacterial peritonitis;only one elderly patient died for infectious shock. Conclusion Anastomotic leakage was the severe complication of low anterior resection,and timely correct treatment and doing well safety precautionary measures before operation could reduce the incidence rate of anastomotic leakage.
