目的:分析我院住院患者发生抗菌药物相关肾损害的情况,以促进临床合理用药。方法:对我院2000年1月-2011年l2月间住院患者发生的抗菌药物相关肾损害的36例病历资料进行回顾性分析,分析抗菌药物相关肾损害发生的可能原因。结果:临床使用范围较广的β-内酰胺类、氨基糖苷类、喹诺酮类、多肽类、多烯类、抗结核药等抗菌药物均可引起肾损害。肾损害多发生在年龄较大患者、长期用药超疗程患者、不按药品说明书超剂量用药患者中。结论:剂量过大、疗程过长是用药后出现肾损害的主要原因;另外,患者年龄状况也是临床抗菌药物相关肾损害发生的重要原因。因此,合理用药、注意对老年肾功能衰退的患者进行密切监测,是预防抗菌药物相关肾病发生的关键。

목적:분석아원주원환자발생항균약물상관신손해적정황,이촉진림상합리용약。방법:대아원2000년1월-2011년l2월간주원환자발생적항균약물상관신손해적36례병력자료진행회고성분석,분석항균약물상관신손해발생적가능원인。결과:림상사용범위교엄적β-내선알류、안기당감류、규낙동류、다태류、다희류、항결핵약등항균약물균가인기신손해。신손해다발생재년령교대환자、장기용약초료정환자、불안약품설명서초제량용약환자중。결론:제량과대、료정과장시용약후출현신손해적주요원인;령외,환자년령상황야시림상항균약물상관신손해발생적중요원인。인차,합리용약、주의대노년신공능쇠퇴적환자진행밀절감측,시예방항균약물상관신병발생적관건。
Objective: By analyzing the information of the inpatients who had antibiotic-associated kidney damage in our hospital, this manuscript tried to remind the clinicians having a reasonable antibiotic-usage. Methods: We had a retrospective analysis of the medical records on 36 inpatients who had antibiotic-associated kidney damage during January 2000 to December 2011 in our hospital, and analyzed the possible causes of antibiotic-associated kidney damage. Results: Those antibiotics which had a wide clinical use such as Lactams, Aminoglycosides, Quinolones, Polypeptides, Polyene and Anti-TB drugs etc., can cause kidney damage. And the kidney damage mostly occurred in patients who are older, and/or with long-term and ultra-treatment medication, and patients who do not follow the drug-instruction and had an excessive dose of medication. Conclusion: Excessive doses and long-term medication are the main reasons for kidney damage; In addition, patient's age is also an important reason that antibiotic-nephropathy occurred. Therefore, reasonable usage of antibiotics and close monitoring of older patients with kidney function decline, are the key methods to prevent antibiotic-nephropathy.
