目的探讨原发性阑尾肿瘤的诊断与治疗问题。方法结合国内外文献对1963～1998年收治的41例原发性阑尾肿瘤进行回顾性分析。结果类癌17例，粘液性肿瘤15例，腺癌9例。发病率0.40%。均经手术治疗，术前误诊率97.6%，术中诊断率仅31.7%。结论阑尾肿瘤缺乏特异表现，与阑尾炎症密切相关，误诊率极高。钡灌肠及B超有助于诊断，强调术中冰冻切片的重要性。手术为主要的治疗方法，术式选择是影响预后的关键因素。
목적탐토원발성란미종류적진단여치료문제。방법결합국내외문헌대1963～1998년수치적41례원발성란미종류진행회고성분석。결과유암17례，점액성종류15례，선암9례。발병솔0.40%。균경수술치료，술전오진솔97.6%，술중진단솔부31.7%。결론란미종류결핍특이표현，여란미염증밀절상관，오진솔겁고。패관장급B초유조우진단，강조술중빙동절편적중요성。수술위주요적치료방법，술식선택시영향예후적관건인소。
Objective　To study the diagnosis and treatment of pri mary tumors of the appendix.Methods　41 cases of primary tumors of the appendix encountered in our hospital from 1963 to 1998 were retrospective ly analysed and relative literatures were reviewed.Results　amon g 41 cases,there were 17 cases of carcinoid,15 cases of mucinous tumor and 9 cas es of adenoc arcinoma.The morbidity was 0.40%.All cases underwent operation,the ra te of misdiagnosis was 97.6% before operation,the rate of intraoperative diagnos is was only 31.7%.Conclusions　Appendicular tumors lack particul ar presantation,which presents similar syptoms as appendicular inflammation.The rate of misdiagnosis is extremely high.Barium enema and ultrasonography are help ful for diagnosis.Intraoperative frozen section is very important for diagnosis .Operation is a principle therapy,correct operation method is important for impr oving the prognosis of appendicular tumor.
