目的通过研究老年性白内障及白内障合并眼底病变两组不同类型白内障的多焦视网膜电图(mERG)表现,了解白内障患者视网膜的局部功能状态,尤其是黄斑区的功能,从而为预测白内障术后视功能提供理论依据.方法对192例282只眼白内障术前进行mERG检测,其中老年性白内障136例201只眼,白内障合并眼底病变56例81只眼,并以40只眼作正常对照.全部192例282只眼白内障患者均行超声乳化术联合植入Alcon公司的折叠人工晶状体,用国际标准视力表测得白内障术后视力.结果经统计学分析,得出与正常对照组比较老年性白内障的mERG第1～6环的各项指标多数差异无统计学意义,而白内障合并眼底病变组差异均有统计学意义;术后视力在0.5以上的mERG第1环PI波振幅密度差异无统计学意义,而术后视力在0.5以下的则差异有统计学意义.结论 mERG可以反映所检测白内障眼的视网膜局部功能状态,对于预测白内障术后视力恢复具有重要参考价值.

Objective To study local retina function in cataract, especially macular area by the different mERG in senile cataract and cataract combined with fundus pathologic lesions.Methods The visual acuity and mERG were examined on 40 eyes of normal control group and 282 eyes of 192 cataract patients.In which, 201 eyes of 136 cases were senile cataract, and 81 eyes of 56 cases were cataract with fundus pathologic lesions.Phacoemulsification and intraocular lens implantation was done on all patients.Results Each index from 1 to 6 loop of mERG had no obvious differences between control and senile cataract group, but there was significant difference in cataract with fundus pathologic lesions.Swing density of p1-wave in 1 loop was not significant in statistics in those acuity >0.5, whereas there was significant in those acuity ≤ 0.5.Conclusions Local retina status of cataract eyes were reflected by mERG, especially mERG has important role to forecast the visual recovery after operation.
