目的 探讨单核/巨噬细胞在下肢静脉性溃疡的表达及其意义.方法 将下肢慢性静脉功能不全患者28例分为溃疡组和无溃疡组.采用血细胞计数、流式细胞技术、逆转录-聚合酶链反应(RT-PCR)和免疫组织化学方法 检测不同体位下肢外周血和局部皮肤单核/巨噬细胞的表达.另选10例健康志愿者为对照组.结果 各组间两两比较,下肢外周血单核细胞计数和CD68阳性细胞率比较差异无统计学意义(P>0.05);溃疡组平卧位、下垂位血液中CD68阳性单核/巨噬细胞mRNA相对值分别为(0.81±0.21)和(0.66±0.24);皮肤中阳性细胞数为(60.31±14.20)、(58.10±16.23);与无溃疡组和对照组比较,差异均有统计学意义(P<0.05);各组间不同体位之间单核/巨噬细胞免疫组织化学胞阳性细胞数和含量比较,差异无统计学意义(P>0.05).结论 单核/巨噬细胞浸润与下肢静脉性溃疡的发生密切相关.

Objective To investigate the expression and significance of monocyte/macrophage in venous ulceration. Methods Twenty-eight cases of chronic venous insufficiency were divided into ulceration group, non-ulceration group and control group. The expression of monocyte/macrophage was analyzed by blood routine, flow cytometry, reverse transcription-polymerase chain reaction (RT-PCR) and immunohistochemistry in different positions. Ten health volunteers served as controls. Results There was no significant difference in blood monocyte/macrophage among groups. In ulceration group, monocyte/macrophage mRNA: (0. 81 ±0. 21 ), (0. 66 ± 0. 24), and the positive cells in skin were (60. 31 ± 14. 20),(58. 10 ± 16. 23), showed significant difference as compared with the other two groups (P <0. 05). There was no significant difference between different positions ( P > 0. 05 ). Conclusion There is a close correlation between monocyte/macrophage infiltration and venous ulceration of lower limbs.
