目的 探讨CT影像在肺泡蛋白沉积症(PAP)临床疗效判定中的价值.方法 回顾性分析9例PAP患者肺泡灌洗前后胸部CT影像资料、肺功能检查结果及血气分析检测指标;在CT图像上对病变范围进行影像学量化评分,并与肺功能及血气分析指标进行相关性分析.结果 9例PAP患者肺泡灌洗前胸部CT具有特征性影像表现,病变范围量化评分与肺总量(TLC)、用力呼吸肺活量(FVC)、一氧化碳弥散量(D_LCO)、动脉血氧分压(PaO_2)和动脉血氧饱和度(SaO_2)呈显著负相关,P值均<0.05.肺泡灌洗后肺功能各项指标均有不同程度改善,PaO2、SaO2较灌洗前明显增加.结论 胸部CT检查可为PAP临床诊断、治疗及预后评估提供客观依据.

Objective To study the value of chest CT in evaluating the curative effect for pulmonary alveolar proteinosis (PAP). Methods The chest CT findings and the results of pulmonary function tests and blood gas analysis in 9 patients with PAP before and after bronchoalveolar lavage (BAL) were reviewed retrospectively. The lesions in extent were scored on CT images. The relationship between the imaging scores and pulmonary function tests as well as blood gas were analysed. Results The results of pulmonary function tests such as TLC,PVC,PEV_1 and D_LCO were improved to a certain degree after BAL in 9 patients with PAP; PaO_2 and SaO_2 were increased significantly after BAL(P<0. 05). CT scores before BAL correlated significantly with TLC , PVC , D_LCO , PaO_2 and SaO_2 (P<0. 05). Conclusion CT examination can provide objective imformations for clinical diagnosis, treatment and prognostic evaluation of pulmonary alveolar proteinosis.
