背景:脑干听觉诱发电位(brainstem auditory evoked potentials,BAEP)和耳蜗电图(electrocochleogram,ECochG)两者综合波的新记录方法具有特殊临床意义,探讨体温升高和体温降低对其的影响对进一步明确ECochG-BAEP综合波的生理特性和指导其临床应用有重要意义.目的:观察体温升高和体温降低对ECochG-BAEP综合波的影响效应.设计:随机区组设计.单位:暨南大学医学院生理教研室.对象:实验于2002-07/09在暨南大学医学院生理学教研室进行,选择成年豚鼠(300~350 g),雌雄不拘,由第一军医大学动物实验中心提供,随机分为体温降低组(20只)和体温升高组(20只).干预:体温降低组,体表物理降温法逐步降低豚鼠体温;体温升高组,体表物理升温法逐步升高豚鼠体温.主要观察指标:ECochG-BAEP综合波的波形、波峰潜伏期(PL)、波峰间潜伏期(IPL)和波幅随体温发生的变化.结果:随体温降低(36~25℃)和体温升高(36~42℃),ECochG-BAEP综合波波形始终兼具BAEP和ECochG两者的特点,有十分明显突出的1波;各波PL和IPL随体温降低而逐步延长,但随体温升高而逐渐缩短;1,2,3波波幅在体温降低至30~29℃时开始出现显著降低,1,2波波幅也在体温升高至40℃后开始出现显著降低.结论:体温降低和体温升高对ECochG-BAEP SW的PL,IPL和波幅均有显著影响,但其波形始终兼具BAEP和ECochG两者的特点.

BACKGROUND: A new method for recording the synthetic wave of electrocochleogram-brain stem auditory evoked potentials(ECochG-BAEP SW)has special clinical significance. The study of effects of hypothermia and hyperthermia on the synthetic wave is significant to define its physiological properties and to guide its clinical application.OBJECTIVE: To observe the effects of hypothermia and hyperthermia on ECochG-BAEP SW.DESIGN: A randomized block design.SETTING: Department of Physiology, Medical College of Jinan University.PARTICIPANTS: The study was performed in the Department of Physiology, Medical College of Jinan University. Adult guinea pigs of either gender weighting 300 - 350 g, provided by the Animal Center of the First Military Medical University of Chinese PLA, were randomly divided into hypothermia group( n = 20) and hyperthermia group( n = 20).INTERVENTIONS: The body temperature of the guinea pigs was decreased step by step with their body contacted with a metal tray filled with ice in hypothermia group, and the body temperature of them was increased stepwise with their body contacted with a hot steel box filled with 50 ℃ circulating water in hyperthermia group.MAIN OUTCOME MEASURES: The waveforms, peak latencies (PL), interpeak latencies(IPL) and wave amplitudes of ECochG-BAEP SW had been observed as the body temperature changed gradually.RESULTS: The waveforms of ECochG-BAEP SW had the characteristics of both BAEP and ECochG, and wave 1 was the most obvious, with body temperatures decreased from 36 ℃ to 25 ℃ and increased from 36 ℃ to 42 ℃. The PL and IPL of ECochG-BAEP SW prolonged more and more as the body temperature was decreased stepwise, and shortened more and more as the body temperature was increased gradually. The amplitudes of wave 1, 2 and 3 of ECochG-BAEP SW became significant smaller as the body temperature was decreased to 30 ℃ or 29 ℃, and the amplitudes of wave 1 and 2 were decreased significantly as the body temperature was increased to 40 ℃.CONCLUSION: The effects of hypothermia and hyperthermia on PL, IPL and wave amplitudes of ECochG-BAEP SW were significant, but its waveforms always had the characteristics of both BAEP and ECochG.
