目的分析肾移植受者术后发生恶性肿瘤的情况.并探讨发生恶性肿瘤的危险因素和预后.方法回顾性分析1991年1月至2007年8月间2392例肾移植受者中术后恶性肿瘤的发生率、恶性肿瘤的类型、移植时年龄、术后至肿瘤诊断的时间、免疫抑制剂应用情况及预后等资料;比较免疫诱导组与非免疫诱导组的一般情况;并进行发生肿瘤危险因素的相关性分析.结果 31例肾移植受者术后发生恶性肿瘤,发生率为1.29%.恶性肿瘤发生率与肾移植时年龄和免疫诱导治疗呈正相关.免疫诱导组在恶性肿瘤诊断时肾功能不全发生率较非免疫诱导组明显降低,但肾移植术后至恶性肿瘤诊断时的时间明显缩短,且患者术后1年存活率无明显改善.88.7%的患者于诊断后24个月内死亡.结论肾移植时的年龄和免疫诱导是发生恶性肿瘤的危险因素.肾移植术后发生恶性肿瘤的患者预后差.

Objective To analyze the incidence of malignant tumor and investigate the risk fac-tors and prognosis in kidney transplant recipients. Methods The clinical data of 2392 kidney trans-plant cases in our hospital between Jan. 1991 and Aug. 2007 were retrospectively analyzed on the type of malignant tumor, the age of recipients at the time of transplantation, the time interval between transplantation and tumor detection, the type of immunosuppressant after kidney transplantation, the strategy of treatment and the prognosis. The general conditions were compared between group of in-ductive treatment and group of non-inductive treatment. The correlative analysis was done on tumor-related risk factors. Results The incidence of malignant tumor was 1.29% in kidney transplant reci-pients, and it was positively correlated with the age of recipients at the time of transplantation and in-ductive treatment with monoclonal antibodies. The incidence of the renal dysfunction in the recipients who were induced with ALG/Daelizumab was significantly lower than that in the non-induced reci-pients, but the time interval between transplantation and tumor detection was significantly shorter, and the 1-year survival rate was not obviously improved. 88.7 % of the deceased cases were died with-in 24 months after malignant tumor diagnosis. Conclusions Induction with monoclonal antibodies and the age of recipients at the time of transplantation were risk factors of malignant tumor development. The prognosis of the recipients suffering from malignant tumor was poor.
