目的 观察去骨瓣减压术对大鼠缺血缺氧性脑损伤后核因子(NF)-κB表达及细胞凋亡的影响.方法 将60只SD大鼠随机分3组:脑缺血(IS)组、脑缺血+去骨瓣减压(IS+DC)组、去骨瓣减压(DC)组,每组20只.不同时间点取脑,三苯基氯化四氮唑(TTC)染色观察脑梗死范围,电镜观察半暗区神经元超微结构变化,免疫组织化学测定NFκB表达,TUNEL法检测细胞凋亡变化.结果 与单纯脑缺血组比较,(IS+DC)组脑梗死范围减小,半暗区神经元损伤减轻,NF-κB表达降低(0.0621±0.0101,P<0.05),细胞凋亡数量减少(10.93±3.54,P<0.05).结论 去骨瓣减压术通过降低缺血半暗区脑组织NF-κB基因表达,减少细胞凋亡,起到脑保护作用.

목적 관찰거골판감압술대대서결혈결양성뇌손상후핵인자(NF)-κB표체급세포조망적영향.방법 장60지SD대서수궤분3조:뇌결혈(IS)조、뇌결혈+거골판감압(IS+DC)조、거골판감압(DC)조,매조20지.불동시간점취뇌,삼분기록화사담서(TTC)염색관찰뇌경사범위,전경관찰반암구신경원초미결구변화,면역조직화학측정NFκB표체,TUNEL법검측세포조망변화.결과 여단순뇌결혈조비교,(IS+DC)조뇌경사범위감소,반암구신경원손상감경,NF-κB표체강저(0.0621±0.0101,P<0.05),세포조망수량감소(10.93±3.54,P<0.05).결론 거골판감압술통과강저결혈반암구뇌조직NF-κB기인표체,감소세포조망,기도뇌보호작용.
Objective To investigate the effect of decompressive craniectomy on the expression of NF-κB and apoptosis in the brain of rats with hypoxic-ischemic brain damage.Methods Sixty adult sprague-dawley rats were randomly divided into 3 groups(n=20,each):cerebral isehemia group(IS),cerebral ischemia and decompressive cranieetomy group(IS+DC),and decompressive craniectomy group (DC).The brains were collected to observe the scope of cerebral infarction by TTC staining,to examine the uhrastrueture of ischemic penumbra by an electronic microscope,to detect the expression of NF-κB by immunohistochemistry and to test the apoptosis by TUNEL.Results Compared with the other groups,the infarction volume was decreased,the damage of ischemic penumbra neurons was relieved.the expression of NF-κB was attenuated remarkably(0.0621±0.0101,P<0.05),and the apoptotie cells were decreased significantly(0.0621±0.0101,P<0.05)in IS+DC group.Conclusion Deeompressive craniectomy may protect the brain by increasing the expression of NF-κB and decreasing the apoptotie cells in cerebral iscbemie penumbra.
