目的 观察缺血后适应干预措施能否减轻兔急性肠系膜缺血再灌注损伤.方法 将雄性新西兰兔120只随机分为空白对照(Con)组(仅开腹显露SMA)、缺血再灌注(I/R)组[肠系上动脉(superior mesenteric artery,SMA)缺血30 min后持续灌注120 min]、缺血后适应1(IpostC1)组(SMA缺血30 min后行3个循环的灌注30 s/阻断30 s,再持续灌注117 min)、缺血后适应2(IpostC2)组(SMA缺血30 min后行3个循环的灌注60s/阻断60 s,再持续灌注114 min),每组30只.干预后采集各组兔处理后下腔静脉血及部分肠道组织标本,试剂盒测定血清及肠道组织内丙二醛(MDA)、髓过氧化酶(MPO)水平,HE染色,Chiu 6级评分法观察肠黏膜损伤情况. 结果 与对照组比较,I/R组及IpostC1组、IpostC2组血清及肠道组织中MDA、MPO水平明显增加(P<0.01),肠黏膜损伤评分明显升高(P<0.01).与I/R组相比,IpostC1组血清和肠道组织中MDA、MPO水平及肠黏膜损伤评分明显降低(P<0.01),而IpostC2组血清和肠道组织中MDA、MPO水平及肠黏膜损伤评分无明显降低(P>0.05).结论 缺血后适应可明显降低兔急性肠系膜缺血再灌注损伤后下腔静脉血和肠道组织中MDA、MPO水平及肠黏膜损伤.

목적 관찰결혈후괄응간예조시능부감경토급성장계막결혈재관주손상.방법 장웅성신서란토120지수궤분위공백대조(Con)조(부개복현로SMA)、결혈재관주(I/R)조[장계상동맥(superior mesenteric artery,SMA)결혈30 min후지속관주120 min]、결혈후괄응1(IpostC1)조(SMA결혈30 min후행3개순배적관주30 s/조단30 s,재지속관주117 min)、결혈후괄응2(IpostC2)조(SMA결혈30 min후행3개순배적관주60s/조단60 s,재지속관주114 min),매조30지.간예후채집각조토처리후하강정맥혈급부분장도조직표본,시제합측정혈청급장도조직내병이철(MDA)、수과양화매(MPO)수평,HE염색,Chiu 6급평분법관찰장점막손상정황. 결과 여대조조비교,I/R조급IpostC1조、IpostC2조혈청급장도조직중MDA、MPO수평명현증가(P<0.01),장점막손상평분명현승고(P<0.01).여I/R조상비,IpostC1조혈청화장도조직중MDA、MPO수평급장점막손상평분명현강저(P<0.01),이IpostC2조혈청화장도조직중MDA、MPO수평급장점막손상평분무명현강저(P>0.05).결론 결혈후괄응가명현강저토급성장계막결혈재관주손상후하강정맥혈화장도조직중MDA、MPO수평급장점막손상.
Objective To study the effect of ischemia postconditioning intervention in a rabbit's acute mesenteric ischemia-reperfusion injury model. Methods 120 rabbits were divided randomly into Con( only expose SMA by operation),I/R( clamping SMA 30 min,reperfusing 120 min),IpostC1 ( clamping SMA 30 min,3 clamping 30 s/releasing 30 s round,reperfusing 117 min),and IpostC2 (clamping SMA 30 min,3 clamping 60 s/releasing 60 s round,reperfusing 114 min) group (n =30).Levels of MDA and MPO in serum and intestinal tissues were measured. Chiu-6 standard scoring was used to determine the pathology score of injured intestinal mucosae. Results Compared with the Con group,MDA and MPO levels in serum and intestinal tissues increased obviously in the three other groups,the same as in the pathology score of injured intestinal mucosae (P < 0.01 ) ; Compared with the I/R group,the MDA and MPO levels in serum and intestinal tissues decreased obviously in the IpostC1 group ( P < 0.01 ),but not in the IpostC2 group ( P > 0.05 ). Conclusions MDA and MPO levels in serum and intestinal tissues and intestinal mucosal injury decreased obviously in the rabbit's acute mesenteric ischemia-reperfusion injury model by ischemia postconditioning intervention.
