目的 评估3D-DSA在颅内动脉瘤夹闭术后检测残留动脉瘤中的利用价值.方法 对29例已实施动脉瘤夹闭术的颅内动脉瘤(35个动脉瘤)患者,在术后3-6个月内行3D-DSA及DSA检查,评估3D-DSA对残留动脉瘤的检出效果并与DSA结果进行比较.结果 3D-DSA检出残留动脉瘤共10个,其中8个瘤颈残留动脉瘤为术中已经确认瘤颈残留的动脉瘤,其他2个瘤体残留动脉瘤为术中不能确认是否完全夹闭的动脉瘤.而DSA共检出残留动脉瘤3个.其中1个瘤颈残留动脉瘤及2个瘤体残留动脉瘤与3D-DSA结果符合,而其他残留动脉瘤均表现为假阴性结果.结论 3D-DSA对动脉瘤夹闭术后残留动脉瘤的检出效果明显优于DSA,尤其对较小瘤颈残留动脉瘤具有较高的敏感度和准确性,故有助于术后长期随访观察残留动脉瘤的形态变化.
目的 評估3D-DSA在顱內動脈瘤夾閉術後檢測殘留動脈瘤中的利用價值.方法 對29例已實施動脈瘤夾閉術的顱內動脈瘤(35箇動脈瘤)患者,在術後3-6箇月內行3D-DSA及DSA檢查,評估3D-DSA對殘留動脈瘤的檢齣效果併與DSA結果進行比較.結果 3D-DSA檢齣殘留動脈瘤共10箇,其中8箇瘤頸殘留動脈瘤為術中已經確認瘤頸殘留的動脈瘤,其他2箇瘤體殘留動脈瘤為術中不能確認是否完全夾閉的動脈瘤.而DSA共檢齣殘留動脈瘤3箇.其中1箇瘤頸殘留動脈瘤及2箇瘤體殘留動脈瘤與3D-DSA結果符閤,而其他殘留動脈瘤均錶現為假陰性結果.結論 3D-DSA對動脈瘤夾閉術後殘留動脈瘤的檢齣效果明顯優于DSA,尤其對較小瘤頸殘留動脈瘤具有較高的敏感度和準確性,故有助于術後長期隨訪觀察殘留動脈瘤的形態變化.
목적 평고3D-DSA재로내동맥류협폐술후검측잔류동맥류중적이용개치.방법 대29례이실시동맥류협폐술적로내동맥류(35개동맥류)환자,재술후3-6개월내행3D-DSA급DSA검사,평고3D-DSA대잔류동맥류적검출효과병여DSA결과진행비교.결과 3D-DSA검출잔류동맥류공10개,기중8개류경잔류동맥류위술중이경학인류경잔류적동맥류,기타2개류체잔류동맥류위술중불능학인시부완전협폐적동맥류.이DSA공검출잔류동맥류3개.기중1개류경잔류동맥류급2개류체잔류동맥류여3D-DSA결과부합,이기타잔류동맥류균표현위가음성결과.결론 3D-DSA대동맥류협폐술후잔류동맥류적검출효과명현우우DSA,우기대교소류경잔류동맥류구유교고적민감도화준학성,고유조우술후장기수방관찰잔류동맥류적형태변화.
Objective The aim of this study was to assess the values of detecting intracranial residual aneurysms by three - dimensional digital subtraction angiography (3D - DSA) after clipping. Methods 3D - DSA and DSA were performed on 29 patients (35 aneurysms) after 3 to 6 months of clipping to assess the effect of detecting residual aneurysms by 3D - DSA contrasting to DSA. Results 10 residual aneurysms were detected by 3D - DSA, in which 8 neck residual aneurysms had been confirmed as the neck residual aneurysms, the other 2 residual dome aneurysms could not be confirmed as the completely clipped aneurysms in operations; However, only 3 aneurysms were detected by DSA, in which 1 neck residual aneurysm and 2 residual dome aneurysms were in line with the results of3D -DSA. While the other 7 residual aneurysm, which were detected by 3D - DSA, appeared as false negative results. Conclusion The effect of detecting residual aneurysms after clipping by 3D - DSA was obviously better than by DSA. 3D -DSA was more sensitive and accurate, especially in detecting the less neck remnant aneurysms. 3D - DSA was good at observing the morphologic change of residual aneurysms in long - term follow - up after clipping.
