目的探讨肛门镜下吻合器痔切除术(PPH)的临床应用价值。方法分析重度内痔(Ⅲ度、Ⅳ度内痔)经吻合器切除的诊治资料。结果经肛门镜下采用33mm吻合器切除重度内痔36例，平均手术时间10min，术后平均住院3.5天，6例术后第2～7天有便血，经保守治疗后好转。12例术后当天无肛门疼痛。随访1～5个月效果良好。结论经肛门镜下吻合器痔切除术是一种新技术，具有安全、有效、住院时间短和恢复快的优点，有望替代传统的手术治疗方法。
목적탐토항문경하문합기치절제술(PPH)적림상응용개치。방법분석중도내치(Ⅲ도、Ⅳ도내치)경문합기절제적진치자료。결과경항문경하채용33mm문합기절제중도내치36례，평균수술시간10min，술후평균주원3.5천，6례술후제2～7천유편혈，경보수치료후호전。12례술후당천무항문동통。수방1～5개월효과량호。결론경항문경하문합기치절제술시일충신기술，구유안전、유효、주원시간단화회복쾌적우점，유망체대전통적수술치료방법。
Objective　To evaluate the efficacy of the stapled prolapsectomy(PPH) in treatment of third or fourth degree hemorrhoids.Methods　Analyze the date of the third or fourth degree hemorrhoids operated by PPH.Results　Total of a 33mm circular staplar,mean surgical time was 10 minutes,mean operation stay were 3.5 days,postoperative bleeding was noted in 6 cases (16.7%) at the day of 2rd and 7th day of the operation,they were cured by conservative method.12 patients had no pain at the day of the operation.Follow up for 1～5 months the treatment of effective.Conclusion　The new technique of PPH was a safe and effective treatment for third or fourth degree hemorrhoids and with the advantages of shorter time of inpatient stay and earlier return to normal activity,it may be an alternative for the conventional haemorrhoidectomy.
