目的通过多导睡眠图(PSG)及临床指标预测阻塞型睡眠呼吸暂停综合征患者(OSAS)的经鼻持续气道正压通气(nCPAP)最佳压力(Pmin)。方法采用RHK-5500型多导睡眠呼吸监测系统连接BiPAP系统测22例OSAS患者nCPAP的Pmin,推导其与呼吸紊乱指数(RDI)、体重指数(BMI)及颈围(NECK)等指标的回归方程,用回归方程预测的Pmin治疗20例OSAS患者,同时多导睡眠图监测治疗效果。结果 RDI、LSaQ2指标与Pmin呈直线正相关。结论临床上可根据患者的RDI、LSaO2预测nCPAP治疗的最佳压力。

Objective To predict an optimal level of continuous positive airway pressure (CPAP) in the management of obstructive sleep apnea syndrome (OSAS) by relating certain parameters of respiratory disturbance and sleep hypoxcmia. Methods 42 patients with OSAS were enrolled in the study. The actual levels of CPAP (Pmin) in Group one (22 patients, aged 54. 3 ± 12. 6yrs ) were determined by a RHK-5500 mode polysomnographic system plus BiPAP (S-T/D) system. The actual Pmin was related to the respiratory disturbance index (RDI). The correlation regression equations were calculated. 20 patients with OSAS (Group two, aged 50. 1 ± 11.8 yrs) were treated with the predicted nasal CPAP which was derived from the regression equation. Results There was a close postive linear correlation between RDI or LSaO2 and actual Pmin. Symptoms and polysomnographic parameters improved significantly after the CPAP treatment in these patients. The efficacy showed no significant difference between the two groups. Conclusions It was suggested that RDI is of value and simple in predicting the pressure level of CPAP in the management of OSAS with nasal CPAP.
