目的 探讨血清肿瘤标志物细胞角蛋白21-1(CYFRA21-1)、神经元特异性烯醇化酶(NSE)、癌胚抗原(CEA)、鳞癌相关抗原(SCCAg)联合检测对肺癌诊断的临床应用价值.方法 采用电化学发光法检测203例肺癌患者(肺癌组)、100例肺部良性病变患者(良性对照组)和50例健康体检者(正常对照组)血清中的四种肿瘤标志物,并用统计学方法比对分析.结果 肺癌患者组血清四种肿瘤标志物水平明显高于良性对照组和正常对照组(P<0.01),CYFRA21-1敏感性为70.9%,特异性为94.0%;NSE敏感性为36.5%,特异性为 92.0%;CEA敏感性为46.8%,特异性为90.0%;SCCAg敏感性为46.3%,特异性为95.0%;四项联合检测可大大提高肺癌诊断的敏感性,敏感性高达95.1%,特异性为82.0%.结论 联合检测血清中的CYFRA21-1、NSE、CEA、SCCAg可有效提高肺癌诊断的敏感性,对肺癌的早期诊断有重要临床应用价值.

Objective To investigate the clinical value of combined detection of serum tumor makers CYFRA21-1, NSE, CEA and SCCAg in diagnosis of lung cancer. Methods To detect serum tumor markers of 203 patients with lung cancer (lung cancer group), 100 patients with benign lesions(benign control group)and 50 cases of physical examination persons (normal control group) with electrochemical luminescence method, it was statistical analyzed retrospectively.Results The four kinds of serum tumor makers in lung cancer group were obviously higher than bengin control group and normal control group (P<0.01). The sensitivity of CYFRA21-1 was 70.9%, specificity was 94.0%.The sensitivity of NSE was 36.5%, specificity was 92.0%.The sensitivity of CEA was 46.8%, specificity was 90.0%. The sensitivity of SCCAg was 46.3%, specificity was 95.0%. The sensitivity of combined detection was higher than any of them, the sensitivity of the four combined detections was 95.1%, specificity was 82.0%. Conclusions Combined detection of serum markers may effectively improve the sensitivity of lung cancer diagnosis and it has important value of clinical applications for early diagnosis of lung cancer.
