目的 探讨经皮激光汽化减压联合臭氧髓核氧化术治疗椎间盘源性腰痛的临床疗效.方法 应用医用半导体激光和医用臭氧,在X线引导下采用18G穿刺针经皮穿刺至髓核区,然后由穿刺针导入600μm光导纤维,用脉冲式激光切割、汽化髓核,以每个椎间盘800~1000J切割、汽化,再向椎间盘注入浓度为60μg/ml臭氧约5~8ml,共治疗74例95节腰椎间盘.结果 按照MACNAB标准评定,74例患者术后随访5~36个月,优64例,良6例,可4例,差0例,优良率94.6%.结论 经皮激光汽化减压联合臭氧髓核氧化术治疗椎间盘源性腰痛,疗效佳,操作简便,创伤小,安全性高,恢复快.

목적 탐토경피격광기화감압연합취양수핵양화술치료추간반원성요통적림상료효.방법 응용의용반도체격광화의용취양,재X선인도하채용18G천자침경피천자지수핵구,연후유천자침도입600μm광도섬유,용맥충식격광절할、기화수핵,이매개추간반800~1000J절할、기화,재향추간반주입농도위60μg/ml취양약5~8ml,공치료74례95절요추간반.결과 안조MACNAB표준평정,74례환자술후수방5~36개월,우64례,량6례,가4례,차0례,우량솔94.6%.결론 경피격광기화감압연합취양수핵양화술치료추간반원성요통,료효가,조작간편,창상소,안전성고,회복쾌.
Objective To explore the therapeutic effect of treatments with Percutaneous Laser Disk Decompression( PLDD ) combining ozone nucleus gelatinosus oxidizing technique in Discogenic Low Back Pain ( DLBP ). Methods The spinal needle was inserted in the space of the nucleus gelatinosus under X-ray guidance.74 patients with 95 lumbar disks were treated by pulsed-wave mode laser cutting and vaporizing ( with the power of 600 ~ 1000 J for each lumbar disk ) and O3( 5~8 ml with concentration of 60μ g/ml influxed ).Results According to Macnab's standard, among the total 74 patients with the followup of 5 to 36 months, 64 patients had excellent therapeutic effects, 6 patients had good therapeutic effects,4 patients had some therapeutic effects and no one had no therapeutic effects. The total excellent and good effective rate is 94.6%. Conclusion The PLDD combining ozone nucleus gelatinosus oxidizing technique treats DLBP, with advantages of good therapeutic effect, easy operation, little injury, high safety and fast recovery.
