目的 探讨喉罩通气道替代面罩和气管插管供氧,治疗老年患者开胸手术后低氧血症的疗效.方法 手术全麻清醒后拔管,在鼻氧管吸氧状态下又出现中重低氧血症持续5 min的60例病人,平均分为A组喉罩供氧,B组面罩供氧,C组气管插管供氧,每组各20例,A组采用一次性普通3号或4号喉罩,置入喉罩到正常位置,接呼吸机或麻醉机行自主呼吸或间歇指令呼吸.B组行面罩供氧,C组行气管插管控制呼吸供氧.观察喉罩、面罩、气管插管通气1、3、5、10 min各时点脉搏血氧饱和度(SpO2)的变化及副作用的发生.结果 喉罩、气管插管较面罩通气1、3 min时点脉搏血氧饱和度(SpO2)的无明显变化,在5、10 min时点时差异有统计学意义.A、B组均有胃胀气、反流、误吸等副作用发生.气管插管组咽疼干的症状较多.结论 喉罩通气应用于低氧血症能达到有效通气的作用.

목적 탐토후조통기도체대면조화기관삽관공양,치료노년환자개흉수술후저양혈증적료효.방법 수술전마청성후발관,재비양관흡양상태하우출현중중저양혈증지속5 min적60례병인,평균분위A조후조공양,B조면조공양,C조기관삽관공양,매조각20례,A조채용일차성보통3호혹4호후조,치입후조도정상위치,접호흡궤혹마취궤행자주호흡혹간헐지령호흡.B조행면조공양,C조행기관삽관공제호흡공양.관찰후조、면조、기관삽관통기1、3、5、10 min각시점맥박혈양포화도(SpO2)적변화급부작용적발생.결과 후조、기관삽관교면조통기1、3 min시점맥박혈양포화도(SpO2)적무명현변화,재5、10 min시점시차이유통계학의의.A、B조균유위창기、반류、오흡등부작용발생.기관삽관조인동간적증상교다.결론 후조통기응용우저양혈증능체도유효통기적작용.
Objective To study the laryngeal mask airway (LMA)replacing face mask and endotracheal intubation which was used for hypoxemia elderly patients after thoracotomy.Methods The endotracheal intubations were removed after the patients fully awake.When obsorbing oxygen through nasal oxygen tube,60 patients who suffered from serious hypoxemia for 5 minutes were divided into group A using LMA,group Busing face mask and group C using endotracheal intubations.20 cases were randomly respectively selected from A,B group.Group A used one -time 3rd or4th classic LMA,and the LMA were placed at the normal location,then connected on the ventilator or anesthesia machine let the patients breathe independently or intermittently mandatorily.Group B used face mask,and group C used LMA to control breathe.The SpO2 and the side effect under the these conditions were observed respectively at 1,3,5,10 minutes.Results The SpO2 of the three group were basically the same at 1,3 minutes, but gastric up gas, reflux, aspiration were happened to the patients of group A and group B at 5,10 minutes and the drying and pain throat was very common in group C.Conclusion From the above we can conclude that LMA can give an effectively ventilation when hypoxemia happen.
