目的观察玻璃体腔注射曲安奈德联合悬吊式人工晶状体植入治疗无晶状体眼合并黄斑水肿的疗效.方法对25例(25只眼)合并黄斑水肿的无晶状体眼患者,给予玻璃体腔内注射0.1 ml(4 mg)曲安奈德,并于3～5大后行悬吊式人工晶状体睫状沟缝线固定植入术,术后随访6个月,观察术后视力、眼压、黄斑区神经上皮层厚度及并发症.结果术后1月平均矫正视力0.28±0.11；术后6个月平均矫正视力0.46±0.16；术后1个月黄斑区神经上皮平均厚度为(226.00±21.65) μm；术后6个月神经上皮平均厚度为(158.00±13.73) μm；与术前相比差异均有统计学意义(P＜0.01).未发现严重的手术并发症.结论悬吊式人工晶状体睫状沟固定植入术可改善无晶状体眼的屈光状态,联合玻璃体腔注射曲安奈德对无晶状体眼黄斑水肿的治疗是一种安全有效的方法.
목적 관찰파리체강주사곡안내덕연합현조식인공정상체식입치료무정상체안합병황반수종적료효.방법 대25례(25지안)합병황반수종적무정상체안환자,급여파리체강내주사0.1 ml(4 mg)곡안내덕,병우3～5대후행현조식인공정상체첩상구봉선고정식입술,술후수방6개월,관찰술후시력、안압、황반구신경상피층후도급병발증.결과 술후1월평균교정시력0.28±0.11；술후6개월평균교정시력0.46±0.16；술후1개월황반구신경상피평균후도위(226.00±21.65) μm；술후6개월신경상피평균후도위(158.00±13.73) μm；여술전상비차이균유통계학의의(P＜0.01).미발현엄중적수술병발증.결론 현조식인공정상체첩상구고정식입술가개선무정상체안적굴광상태,연합파리체강주사곡안내덕대무정상체안황반수종적치료시일충안전유효적방법.
Objective To evaluate the effect of intravitreal injection of triamicinolone acetonide combined with transscleral fixation of IOL for treatment of macular edema of aphakic eyes. Methods A 0.1 milliliter triamicinolone acetonide was injected into vitreous of 25 aphakic eyes combined with macular edema,3-5 days later,all patients had transscleral fixation of IOL.The follow-up time was 6 months,the average intraocular pressure (IOP),visual acuity,the thickness of the lay of nerve fibers of macular and the complications were observed. Results The mean corrective visual acuity was 0.28±0.11 at 1 month after the treatment,0.46±0.16 at 6 months,there was statistically significant differences than pre-treatment (P＜0.01).The mean thickness of macular nerve fibers was 226.00±21.65μm at 1 month after the treatment, 158.00±13.73μm at 6 months after the treatment,there was statistically significant differences than pre-treatment (P＜0.01). Conclusions Implanting transscleral fixation of IOL can improve the refractive acuity,when combined with intravitreal injection of triamicinolone acetonide is a safe and effective way to treat the macular edema of aphakic eyes.
