目的 探讨原发性高血压患者内皮舒张因子[降钙素基因相关肽(CGRP)和前列环素(PGI2)]、内皮收缩因子[血栓烷B2(TXB2)和内皮素(ET)-1]与高敏C反应蛋白(ha-CRP)、可溶性细胞间黏附分子(sICAM)-1的关系.方法 选取未经治疗的原发性高血压患者42例作为高血压组,对照组由40例同期健康体检者组成.采用放免法测定CGRP、6-酮-前列腺素F1α(6-keto-PGF1α)、ET-1及TXB2水平;ELISA法测定sICAM-I水平;激光散射比浊法测定hs-CRP水平.结果 高血压组ET-1、TXB2、ha-CRP、sICAM-1水平[分别为(102.23±17.60)ng/L、(102.25±24.61)ng/L、(14.58±2.16)mg/L、(224.89±49.28)μg/L]显著高于对照组[分别为(50.81±6.54)ng/L、(76.11.±20.23)ng/L、(2.33±1.21)mg/L、(112.51±22.47)μg/L],高血压组CGRP、6-keto-PGF1α水平[分别为(28.95±9.68)、(54.26±18.96)ng/L]显著低于对照组[分别为(63.12±16.74)、(89.54±21.48)ng/L],差异均有统计学意义(P<0.01);高血压组ET-1、TXB2、ha-CRP、sICAM-1水平随着血压分级升高而升高,CGRP、6-keto-PGF1α水平随着血压分级升高而降低,组间比较差异有统计学意义(P<0.05或<0.01);高血压组hs-CRP、sICAM-1水平与ET-1及TXB2水平呈正相关,与CGRP、6-keto-PGF1α水平呈负相关.结论 血浆ET-1、TXB2和血清ha-CRP及sICAM-1水平变化与血压升高有紧密联系,且内皮功能与炎性因子存在相关性,提示它们参与了高血压的发生和发展.

Objective To study the relationship between endothelial diastolic factor[calcitoningene-related peptide(CGRP),prostaglandin I2(PGI2)],endothelial contract factor[endothelin(ET)-1,thromboxane B2(TXB2)],the serum high-sensitivity C reactive protein(ha-CRP)and soluble intercellular adhesion molecule(sICAM)-1 level in patients with essential hypertension(EH).Methods Forty-two EH patienm(EH group)received no medication were randomly divided into 3 subgroups(class 1,class 2,class 3),and 40 healthy volunteers were enrolled as control group.The levels of CGRP,6-keto-PGF1α,ET-1,TXB2 were determined by the method of radioimmunity,and the serum level of sICAM-1 Was detected by ELISA while the level of hs-CRP was detected by the method of Laser nephelometry.Results The levels of ET-1,TXB2,ha-CRP,sICAM-1 were higher,whereas the levels of CGRP,6-keto-PGF1α were lower in EH group[(102.23±17.60)ng/L,(102.25±24.61)ng/L,(14.58±2.16)mg/L,(224.89±49.28)μg/L,(28.95±9.68)ng/L,(54.26±18.96)ng/L,respectively]than those in control group[(50.81±6.54)ng/L,(76.11 ±20.23)ng/L,(2.33±1.21)mg/L,(112.51±22.47)μg/L,(63.12±16.74)ng/L,(89.54±21.4s)ng/L,respectively](P<0.01).the levels of ET-1,TXB2,hs-CBP,sICAM-1 increased,whiled the CGRP,6-keto-PGF1α levels decreased in order in class 1,class 2 and class 3,and there were statistical differences between any two of the classes(P<0.05 or<0.01).The levels of hs-CRP and sICAM-1 were positively correlated with ET-1, TXB2,but negatively correlated with CGRP,6-keto-PGF1α in EH group.Conclusions The changes of ET-1,TXB2,hs-CBP,sICAM-1 have a close relation with the levels of blood pressure in EH patients,and there are correlations between vascular endothelial function and inflammation factor.These suggest that they may be involved in the genesis and development of hypertension.
