目的 研究正常青年人在隔音和非隔音环境下的瞬态诱发耳声发射(TEOAE)和畸变产物耳声发射(DPOAE),探讨环境噪声对耳声发射的影响,为临床应用提供参考。方法 应用Capella耳声发射仪对正常听力青年人102耳分别在隔音和非隔音环境下记录DPOAE和TEOAE。结果 两种环境下DPOAE的幅值无显著差异,信噪比(S/N-ratio)、叠加次数、扫描时间有显著性差异;TEOAE的幅值、信噪比、相关系数(Corr)、叠加次数、扫描时间均有显著性差异;隔音和非隔音环境下的DPOAE、TEOAE的通过率有显著性差异。结论 环境噪声(55~65dBSpL)对DPOAE幅值(除0.5kHz外)无明显影响,对TEOAE有显著影响;在听力筛选时,尤其是在非隔音环境下应用DPOAE较TEOAE准确率高;不但具有频率特异性,且可明显降低假阳性率。

Objective To study the effect of noise on distortion product otoacoustic emission (DPOAE) and transient evoked otoacoustic emission (TEOAE) of normal hearing youths. Methods DPOAE and TEOAE were recorded in 102 ears of normal hearing youths in a sound-treated environment and a none sound-treated environment (55~65 dBSpL) with Cappella Emission Analyzer. The data were analyzed by SPSS 10.0. Results The amplitude of DPOAE had no significant difference (P>0.05) in the two types of environment, but it had significant difference in TEOAE. The S/N-ratio, time, acceptecl times, Corr and pass-rate showed significant difference in the two types of environment. Conclusion Noise of the environment (55~65 dBSpL) has a clear effect on TEOAE, but less obvious effect on DPOAE. DPOAE is more precise than TEOAE for hearing screening, especially in the none sound-treated environment.
